摘 要:目的探讨无创双水平正压通气(BiPPV)治疗慢性阻塞性肺疾病(COPD)合并急性呼吸衰竭的临床效果。方法选取2018年1~12月我院收治的60例COPD合并急性呼吸衰竭患者作为研究对象,采用随机数字表法分为对照组与试验组,每组各30例。对照组采用常规治疗,试验组在对照组的基础上实施BiPPV治疗。比较两组患者不同时间段的血气分析指标、生理指标及最终治疗结局。结果两组患者治疗前的各血气分析指标及生理指标比较,差异均无统计学意义(P>0.05);两组患者治疗后的各血气分析指标及生理指标均较治疗前有所好转,试验组患者治疗12、48、96h时血二氧化碳分压(PaCO2)水平、呼吸(RR)水平均明显低于对照组,pH、动脉血氧分压(PaO2)、血氧饱和度(SpO2)高于对照组,差异均有统计学意义(P<0.05);试验组患者的治疗有效率明显高于对照组,差异有统计学意义(P<0.05)。结论BiPPV治疗COPD合并急性呼吸衰竭患者的临床效果显著,可以改善血气分析指标,稳定其生命体征,对其预后至关重要,值得在临床推广使用。Objective To discuss the clinical effect of non-invasive bi-level positive pressure ventilation(BiPPV)in the treatment of chronic obstructive pulmonary disease(COPD)complicated with acute respiratory failure.Methods A total of 60 COPD patients with acute respiratory failure treated in our hospital from January to December in 2018 were selected as the subjects,and divided into control group and test group by random number table method,with 30 cases in each group.The control group took the conventional therapy,the test group took BiPPV on the basis of the control group.At the different time periods,blood gas analysis indicators,physiological index and treatment outcomes were compared.Results The blood gas analysis indicators and physiological index between groups showed no statistically significant difference before treatment(P>0.05).After treatment,blood gas analysis indicators and physiological index for two groups were improved;at 12,48 and 96 h of treatment,the values of partial pressure of carbon dioxide(PaCO2),respiratory rate(RR)in the test group were lower than those in the control group,and p H,partial pressure of oxygen(PaO2)and oxygen saturation(SpO2)in the test group were higher than those in control group,the differences were statistically significant(P<0.05).The effective rate of the test group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion BiPPV is clinically proven to be effective for COPD patients with acute respiratory failure,which can improve the blood gas analysis indicators,vital signs and clinical prognosis.It is worthy of clinical promotion and application.
