摘　　要：目的探讨不同空心钉置入方式治疗HaraguchiⅠ型后踝骨折的效果。方法选取2013-06—2016-06间收治的96例HaraguchiⅠ型后踝骨折患者。将接受由胫骨前向后踝置入空心钉的54例患者作为前置组,将接受由后踝向胫骨前方置入空心钉的42例患者作为后置组。比较2组的治疗效果。结果(1)前置组切口延迟愈合1例,后置组2例。(2)2组置入内固定手术时间及置入、取出内固定术中出血量和随访期间的踝关节功能AOFAS评分等指标,差异均无统计学意义(P>0.05)。前置组取出内固定手术时间短于后置组,差异有统计学意义(P<0.05)。结论两种空心钉置入方式治疗HaraguchiⅠ型后踝骨折,均有确切疗效,其中由胫骨前向后踝置入空心钉具有取出内固定时间短的优势。Objective To investigate the clinical effect of anterior and posterior hollow nail placement in the treatment of HaraguchiⅠposterior ankle fracture.Methods Totally 96 patients with Haraguchi type I posterior malleolus fracture were selected.Fifty-four patients who received hollow nails from the anterior to posterior ankles of the tibia were used as the anterior group,and 42 patients who received hollow nails from the posterior malleolus to the front of the tibia were used as the posterior group.Comparison of therapeutic effects between the two groups were made.Results There was significant difference in the operation time between the two groups in the removal of internal fixation(P<0.05).There was no significant difference in intraoperative bleeding,open reduction and internal fixation operation time and postoperative AOFAS score between the two groups.Conclusion Two kinds of hollow nail implantation have definite curative effect in the treatment of Haraguchi type I posterior malleolus fracture,in which the placement of hollow nail from anterior to posterior ankle of tibia has the advantage of short internal fixation time.
