摘 要:目的探讨利多卡因联合多巴胺对机械通气肺损伤的影响。方法选取健康8~12周C57BL/6小鼠32只,按照随机数字表法分为4组:机械通气肺损伤对照组(C组),利多卡因组(L组),多巴胺组(D组),利多卡因联合多巴胺组(LD组),每组各8只。机械通气肺损伤采用Vt 30 ml/kg机械通气4 h制备小鼠机械通气相关肺损伤模型。收集支气管肺泡灌洗液(BAL),检测支气管肺泡灌洗液中蛋白的含量、中性粒细胞数目(PMN)以及肿瘤坏死因子-α(TNF-α)和白细胞介素-10(IL-10)水平。结果与C组(466.70±36.83μg/ml)相比,L组(346.29±29.86μg/ml)、D组(330.95±11.52μg/ml)及LD组(300.61±14.02μg/ml)BAL蛋白含量明显降低(P<0.05);与C组(1.86±0.33×10^5)相比,L组(1.03±0.16×10^5)、D组(0.87±0.07×10^5)及LD组(0.58±0.1×10^5)中性粒细胞计数明显减少(P<0.05);与C组(157.44±27.1 pg/ml)比较,L组(110.35±17.68 pg/ml)、D组(98.54±10.49 pg/ml)及LD组(80.48±6.84 pg/ml)BAL TNF-α含量明显降低(P<0.05)。此外,L组(3.30±0.13 pg/ml)与LD组(3.37±0.244pg/ml)肺泡灌洗液IL-10含量明显增高(P<0.05)。结论利多卡因联合多巴胺能够明显减轻小鼠因机械通气引起的机械通气肺损伤。Objective To investigate the effect of lidocaine combined with dopamine on ventilator-induced lung injury.Methods Thirty-two C57 BL/6 mice were divided into 4 groups using a random number table,including Control group(group C),Lidocaine group(group L),Dopamine group(group D)and Lidocaine combined with Dopamine group(group LD),8 cases in each group.Ventilator-induced lung injury was induced by 4 h mechanical ventilation with tidal volume 30 ml/kg.The bronchoalveolar lavage fluid(BAL)was collected for determination of polymorphonuclear leukocytes(PMN),concentration of protein,tumor necrosis factor-α(TNF-α)and interlukin-10(IL-10).Results Compared with group C(466.70±36.83μg/ml),BAL protein level in group L(346.29±29.86μg/ml),group D(330.95±11.52μg/ml)and group LD(300.61±14.02μg/ml)decreased significantly(P<0.05).Compared with group C(1.86×10^5±0.33×10^5),BAL PMN in group L(1.03×10^5±0.16×10^5),group D(0.87×10^5±0.07×10^5)and group LD(0.58×10^5±0.1×10^5)decreased significantly(P<0.05).Compared with group C(157.44±27.1 pg/ml),BAL TNF-αlevel in group L(110.35±17.68 pg/ml),group D(98.54±10.49 pg/ml)and group LD(80.48±6.84 pg/ml)decreased significantly(P<0.05).In addition,the BAL IL-10 levels increased obviously in group L(3.30±0.13 pg/ml)and group LD(3.37±0.244 pg/ml)(P<0.05).Conclusion Lidocaine combined with dopamine can significantly reduce ventilator-induced lung injury caused by mechanical ventilation.
