抑郁障碍患者人际关系影响因素的横断面研究

摘　　要：目的通过分析抑郁障碍患者人际关系的状态及其影响因素,为临床干预提供参考。方法运用中国抑郁障碍临床特征调查表(NSSD)自评版和医生版对3142例抑郁障碍患者的人际关系状态及影响因素进行横断面研究,并比较两者之间的差异。结果NSSD医生版和自评版抑郁障碍对人际的负面影响比较差异有统计学意义(P<0.05),医生版和自评版对他人拒绝的敏感性比较差异有统计学意义(P<0.05)。NSSD医生版和自评版患者人际关系的负面影响与孤独感、社交减少、对他人拒绝的敏感性均呈正相关(P<0.01),孤独感与社交减少、对他人拒绝的敏感性均呈正相关(P<0.01),社交减少与对他人拒绝的敏感性呈正相关(P<0.01)。结论医生评与患者自评抑郁障碍患者的人际关系损害的严重程度存在不一致,但均受孤独感、社交减退、对他人拒绝的敏感性等因素的影响。Objective To provide reference for clinical intervention by analyzing the condition and influencing factors of interpersonal relationship in patients with depressive disorder.Methods A total of 3 142 patients with depressive disorder were selected and assessed with National Survey on Symptomatology of Depression(NSSD)(both self-assessment version and doctor’s version)to conduct a cross-sectional study on interpersonal relationship status as well as its influencing factors.Results There existed significant differences in the negative effects of depression on interpersonal relationships as well as the sensitivity to others’rejection between self-assessment version and doctor’s version(P<0.05).The negative effects of depression on interpersonal relationships in NSSD of both versions were positively correlated with loneliness,social decline and sensitivity to others’rejection(P<0.01).The loneliness was positively correlated with social decline and sensitivity to others’rejection(P<0.01),and social decline was positively correlated with sensitivity to others’rejection(P<0.01).Conclusion The severity of interpersonal relationship impairment in patients with self-rated depression is different from that in patients with doctor’s evaluation,but it is affected by loneliness,social decline and sensitivity to rejection.
