血清血管生成素样蛋白水平与冠心病患者Gensini积分的相关性分析

摘　　要：目的:观察分析冠心病(CHD)患者血清血管生成素样蛋白3(ANGPTL3)、血管生成素样蛋白4(ANGPTL4)水平变化,并探讨二者水平与Gensini积分关系。方法:选取洪湖市人民医院心血管内科收治的187例接受冠状动脉造影患者为受试对象,按照冠状动脉造影结果分为CHD组95例(至少有1支冠状动脉狭窄≥50%)、对照组92例(冠状动脉造影完全正常);CHD组中单支病变患者34例、双支病变患者31例、3支病变患者30例;根据Gensini积分四分位间距将CHD患者分为Ⅰ组23例(积分≤10分)、Ⅱ组25例(10<积分<33)、Ⅲ组24例(33≤积分≤60)、Ⅳ组23例(积分>60)。收集基线资料,采用ELISA法检测血清ANGPTL3、ANGPTL4水平。结果:CHD组患者吸烟率、高血压者比例、糖尿病者比例及血肌酐、TC、TG、LDL-C水平均明显高于对照组(P<0. 05)。CHD组单支病变、双支病变、3支病变患者血清ANGPTL3、ANGPTL4水平均显著高于对照组(P<0. 05);CHD组双支病变、3支病变患者血清ANGPTL3、ANGPTL4水平及Gensini积分均显著高于单支病变患者(P<0. 05);CHD组3支病变患者血清ANGPTL3、ANGPTL4水平及Gensini积分明显高于双支病变患者(P<0. 05)。不同Gensini积分组血清ANGPTL3、ANGPTL4水平高低依次为:Ⅳ组、Ⅲ组、Ⅱ组、Ⅰ组,两两比较差异均有统计学意义(P<0. 05)。CHD患者血清ANGPTL3、ANGPTL4水平均与Gensini积分呈正相关(P<0. 05)。结论:CHD患者血清ANGPTL3、ANGPTL4水平均明显升高,二者水平与冠状动脉病变严重程度关系密切,为CHD临床防治提供新方向。Objective: To observe and analyze the changes of serum angiopoietin-like protein 3( ANGPTL3) and angiopoietin-like protein 4( ANGPTL4) levels in patients with coronary heart disease( CHD),and to explore their relationships with Gensini score. Methods: 187 patients who received coronary angiography were selected as subjects in Honghu people’s hospital department of cardiovascular medicine,according to the result of coronary angiography,they were divided into 95 patients in CHD group( at least one coronary artery stenosis was ≥50%) and 92 patients in control group( coronary angiography was completely normal);In group CHD,there were 34 cases of single-vessel disease,31 cases of double-vessel disease and 30 cases of three-vessel disease;CHD patients were divided into 23 patients in group I( score ≤10),25 patients in group II( score<33),24 patients in group III( 33≤score ≤60) and 23 patients in group IV( score > 60) according to the quartile distance of Gensini integral. Baseline data were collected and serum ANGPTL3 and ANGPTL4 levels were measured by ELISA. Results: The smoking rate,proportion of hypertension patients,proportion of diabetes mellitus patients,serum creatinine,total cholesterol,triglyceride,and low density lipoprotein cholesterol levels in CHD group were significantly higher than those in control group( P<0. 05). The levels of serumANGPTL3 and ANGPTL4 of patients with single-vessel disease,double-vessel disease and three-vessel disease in CHD group were significantly higher than those in control group( P<0. 05);the levels of serum ANGPTL3,ANGPTL4 and Gensini score of patients with two-vessel disease and three-vessel disease in CHD group were significantly higher than those of patients with single-vessel disease( P<0. 05);the levels of serum ANGPTL3,ANGPTL4 and Gensini score in CHD patients with three-vessel disease were significantly higher than those in patients with two-vessel lesions( P<0. 05).
