血小板参数在肺部常见疾病患者中的变化及临床意义分析

摘　　要：目的探究血小板相关参数在肺部常见疾病(肺部感染、肺癌、慢性阻塞性肺疾病(COPD))患者变化的临床意义。方法选择安徽医科大学第一附属医院2016年10月至2018年9月收入院的肺部感染、肺癌、COPD的住院患者各50例,以性别、年龄相匹配的该院同期健康体检者50例作为对照组,采用Sysmex XE-2100血细胞分析仪检测血小板相关参数:血小板计数(PLT)、血小板压积(PCT)、血小板分布宽度(PDW)、平均血小板体积(MPV)和大血小板比率(P-LCR)。结果将肺部感染、肺癌以及COPD归类为疾病组,疾病组PLT、PCT明显高于对照组(P<0.05),而PDW、MPV、P-LCR两组间无统计学意义(P>0.05)。肺部感染组和肺癌组PLT、PCT均高于对照组与COPD组(P<0.05),而肺部感染组PDW、MPV、P-LCR均低于其他3组(P<0.05)。PLT和PCT的ROC曲线面积AUC均大于0.5,提示PLT和PCT具有诊断疾病的价值。结论血小板参数的测定尤其PLT和PCT的测定对肺部感染、肺癌和COPD可进行辅助鉴别诊断,对病情评估具有一定临床诊断价值。Objective To analyze the change of platelet parameters in the patients with pulmonary infection,lung carcinoma and COPD.Methods 50 cases of the pulmonary infection,50 cases of the lung cancer,50 cases of COPD and 50 cases of healthy individuals admitted to the hospital were recruited in this study.The platelet parameters(blood platelet count(PLT),platelet volume(PCT),platelet distribution width(PDW),mean platelet volume(MPV)and platelet larger cell ratio(P-LCR)were detected by using Sysmex XE-2100.Results PLT and PCT of disease group which is the combination of pulmonary infection,lung cancer and COPD groups were upregulated compared with control group(P<0.05)while there was no statistically significant diffenence in PDW,MPV and P-LCR between two groups(P>0.05).Patients in pulmonary infection and lung cancer group experienced increased levels of PLT and PCT when compared with control group and COPD group(P<0.05).PDW,MPV and P-LCR in pulmonary infection group were down-regulated compared with other 3 groups(P<0.05).The ROC curve areas AUC of both PLT and PCT are greater than 0.5,suggesting that PLT and PCT may have values in diagnosing disease.Conclusion Determination of platelet parameters,especially PLT and PCT,can be used for the differential diagnosis of pulmonary infection,lung cancer and COPD,and they may have certain clinical diagnostic value for disease assessment.
