摘 要:目的制订并评价血糖监测路径在围手术期高血糖患者中实施的效果。方法制订围手术期高血糖患者血糖监测路径,采用便利抽样的方法,选取2018年5-6月在本院泌尿外科、急诊外科住院的345例围手术期患者,通过路径筛查出70例围手术期高血糖患者作为研究对象,实施围手术期高血糖患者的血糖监测路径。结果通过血糖监测路径的实施,围手术期高血糖患者的检出率为7.41%,70例患者均按规范监测血糖,通过规范监测,及时发现高低血糖患者7例,及时处理,无手术停台、无伤口愈合不良或感染发生,专科随访指导落实率达100%,患者专科随访的依从性64.29%,泌尿外科围手术期高血糖患者的平均住院天数由实施路径前的8.1天下降至实施路径后的7.8天。结论应用围手术期高血糖患者血糖监测路径,有利于外科护士协助医师规范监测和管理血糖,及时发现血糖异常,及早干预,降低术后并发症的发生率,缩短患者住院天数。Objective To formulate and evaluate the implementation effect of blood glucose monitoring pathway in patients with perioperative hyperglycemia.Methods To formulate the blood glucose monitoring path of patients with perioperative hyperglycemia,and 345 perioperative patients in the department of urology and emergency surgery during May and June 2018 were selected by convenient sampling method. 70 perioperative hyperglycemia patients were screened through the path and included in the study, and the blood glucose monitoring path was implement on them. Results Through the implementation of blood glucose monitoring path,the detection rate of hyperglycemia patients in the perioperative period was 7. 41%. 70 patients were monitored according to the standard,7 cases of high and low blood sugar were detected in time through standardized monitoring,and timely treatment,no surgical stop,no poor wound healing or infection,the follow-up guidance of specialist follow-up was 100%. The compliance of the specialist follow-up was 64. 29%. The average hospitalization days in the perioperative period of hyperglycemia in the department of urology was reduced from 8.1 days to 7.8 days.Conclusions The application of blood glucose monitoring path in patients with perioperative hyperglycemia is helpful for surgeons to assist doctors to monitor and manage blood sugar,to find abnormal blood sugar in time,to intervene early,to reduce the incidence of postoperative complications and to shorten the number of patients in hospital.
