彩超检查UAPI、MCAPI指数在双胎妊娠胎儿发育畸形诊断中的作用及其对新生儿结局的预测价值研究

摘　　要：目的探究在双胎妊娠胎儿发育畸形的临床诊断中应用彩超检查的效果,并探究该检查方法对新生儿结局的具体预测价值。方法选取2016年11月~2018年12月医院到本院接受诊断的100例双胎妊娠孕妇,均为复杂性双胎妊娠孕妇,均予以超声检查,并仔细分析其脐动脉搏动指数(Umbilical-artery pulsatility index,UAPI)、大脑中动脉搏动指数(middle cerebral artery pulsatility index,MCAPI)。结果 100例双胎妊娠孕妇中检出胎儿异常共16例(16.00%),包括3例TTTS、6例FGR、1例IUFD、6例FSD。异常组的UAPI1指数明显高于正常组,其MCAPI2指数明显低于正常组(P<0.05);异常组的新生儿存活率、另一胎儿足月分娩率均明显低于正常组(P<0.05);异常组母婴的并发症发生率(56.25%)明显高于正常组(28.57%)(P<0.05)。结论在双胎妊娠胎儿发育畸形诊断中采取彩超检查,可通UAPI、MCAPI指数诊断出胎儿发育异常情况,诊断价值及对新生儿结局的预测价值高,值得推广。Objective:To explore the effect of color Doppler ultrasonography in the clinical diagnosis of twin pregnancy fetal malformation,and to explore the specific predictive value of this method for neonatal outcomes. Methods:100 pregnant women with twin pregnancy who were diagnosed in our hospital from November 2016 to December 2018 were examined by ultrasound.Umbilical artery pulsatility index(UAPI)and middle cerebral artery pulsatility index(MCAPI)were carefully analyzed.Results:16 cases(16.00%)of fetal abnormalities were detected in 100 twin pregnant women,including 3 cases of TTTS,6 cases of FGR,1 case of IUFD and 6 cases of FSD. UAPI1 index of abnormal group was significantly higher than that of normal group,MCAPI2 index of abnormal group was significantly lower than that of normal group(P< 0.05);neonatal survival rate of abnormal group and full-term delivery rate of another fetus were significantly lower than that of normal group(P < 0.05);the incidence of complications of abnormal group(56.25%)was significantly higher than that of normal group(28.57%)(P< 0.05).Conclusion:In the diagnosis of twin pregnancy fetal malformation,color Doppler ultrasonography can diagnose fetal malformation through UAPI and MCAPI index. It has high diagnostic value and predictive value for neonatal outcomes. It is worth popularizing.
