摘　　要：目的探讨血清促代谢因子betatrophin水平与2型糖尿病(T2DM)合并颈动脉粥样硬化的关系。方法根据颈动脉彩超结果,将115例T2DM患者分为T2DM合并颈动脉斑块组(A组,60例)和T2DM组(B组,55例)。选取同期健康体检者38例作为对照组(C组)。检测血脂、血糖、betatrophin水平,分析血清betatrophin水平与T2DM患者颈动脉粥样硬化相关影响因素的关系。结果 A、B组BMI、DBP、HbA1c、FBG、TG和betatrophin水平均高于C组,HDL-C均低于C组(P<0.05),且A组血清betatrophin水平高于B组(P<0.05)。相关性分析显示,血清betatrophin水平与SBP、BMI、FBG、HbA1c和TG呈正相关(r=0.167、0.239、0.305、0.549和0.344,P<0.05),与HDL-C呈负相关(r=-0.316,P<0.01)。多元线性逐步回归分析显示,TG和HbA1c是血清betatrophin水平的影响因素(P<0.01)。多因素Logistic回归分析发现,HbA1c和betatrophin是颈动脉粥样硬化的独立影响因素(P<0.01)。结论 T2DM合并颈动脉粥样硬化患者血清betatrophin水平较高,并与血脂和血糖水平密切相关;betatrophin可能是颈动脉粥样硬化的独立影响因素。Objective To investigate the relationship between serum betatrophin level and type 2 diabetes mellitus(T2 DM) combined with carotid atherosclerosis.Methods According to the results of carotid artery color Doppler ultrasound,115 T2 DM patients were divided into two groups of A(T2 DM combined with carotid atherosclerosis,60 cases) and B(T2 DM without carotid atherosclerosis,55 cases).Tirty-eight healthy people were selected as the controls(group C).The levels of blood lipids,blood glucose and betatrophin were detected.The relationship between serum betatrophin level and the influencing factors of carotid atherosclerosis in T2 DM patients was analyzed.Results The levels of BMI,DBP,HbA1 c,FBG,TG and betatrophin were all higher and HDL-C was lower in groups of A and B than those in group C(P<0.05).The level of betatrophin was higher in group A than that in group B(P<0.05).Serum level of betatrophin was positively correlated to that of SBP,BMI,FBG,HbA1 c and TG(r=0.167,0.239,0.305,0.549 and 0.344,P<0.05) and negatively correlated to that of HDL-C(r=-0.316,P<0.01).The multiple linear stepwise regression analysis showed that TG and HbA1 c were the influencing factors of betatrophin(P<0.01).Multivariate Logistic regression analysis showed that betatrophin and HbA1 c were the independent influencing factors of carotid atherosclerosis(P<0.01).Conclusion Serum level of betatrophin is higher in T2 DM patients combined with carotid atherosclerosis,which is closely related to blood lipids and blood glucose.Betatrophin may be an independent factor influencing carotid atherosclerosis.
