摘　　要：目的比较介入治疗与外科手术治疗肝脏外伤的临床疗效及对患者预后的影响。方法选取桂林医学院第二附属医院肝胆胰外科2016年1月至2018年12月收治的122例肝脏外伤患者进行回顾性研究,其中男性79例,女性43例;年龄15~62岁,平均(36.5±6.3)岁。随机分为试验组和对照组各61例,对照组采用单纯外科手术,试验组采用介入治疗,比较两组患者的血肿缩小面积、手术时间、住院时间、临床疗效、愈合情况和并发症情况。结果试验组手术时间、住院时间、愈合时间低于对照组,差异有统计学意义(P<0.05)。试验组血肿面积缩小50%所需时间及血肿完全或近乎完全消失所需时间短于对照组,差异有统计学意义(P<0.05)。试验组并发症发生率(1.6%)低于对照组(23.0%),差异有统计学意义(P<0.05)。结论对于肝脏外伤患者介入治疗可以更快消除血肿,愈合更快,并发症更少。Objective To investigate the clinical efficacy of interventional therapy and surgical treatment for liver trauma and its influence on the late complications of the patients. Methods 122 patients with liver trauma from January 2016 to December 2018 in the Second Affiliated Hospital of Guilin University. There were 79 males and 43 females aged 15-62 years with an average age of 36.5±6.3 years,and they were randomly divided into the test group and the control group(N=61). The control group was operated only and the test group was treated with interventional therapy. The clinical effect, healing condition, operation time, hospitalization time and complications were compared between the two groups. Results The operation time, hospitalization time and healing time of the test group were significantly lower than that of the control group(P<0.05). The time of reduction of 50% hematoma area and the time of complete or nearly complete disappearance of hematoma in the test group were significantly shorter than that of the control group(P<0.05). The incidence of complications in the test group(1.6%) was significantly lower than that of the control group(23.0%)(P<0.05). Conclusion Interventional treatment of liver trauma may eliminate the bleeding and heal quickly, and has fewer complications.
