摘　　要：目的:明确雄激素及其受体在肛周脓肿发生中的作用。方法:2017年5月—2018年5月,行手术治疗的肛周脓肿、肛瘘和混合痔患者196例,分为研究组112例(肛周脓肿和肛瘘),男89例,女23例,平均年龄(37.75±11.2)岁;对照组(混合痔)84例,男43例,女41例,平均年龄(38.32±10.64)岁。观察血雄激素和雌激素水平、肛管齿状线区有无肛腺及肛腺雄激素受体表达情况。结果:血雄激素水平男性各年龄阶段研究组均低于对照组(P<0.05);女性研究组血雄激素水平在30~39岁组低于对照组(P<0.05);男性及女性研究组各阶段雌激素水平与对照组比较差异无统计学意义(P>0.05);研究组肛管齿状线区肛腺阳性率明显少于对照组(P<0.05);研究组肛腺中雄激素受体数量明显多于对照组(P<0.05);结论:男性肛周脓肿患者的血雄激素水平较正常人低,女性变化不大;肛周脓肿患者齿状线附近的肛腺阳性率较正常人低,肛腺中雄激素受体水平代偿性升高;机体存在血雄性激素-雄激素受体-肛腺反应轴,肛腺数量少却分泌旺盛,易导致腺管堵塞,堵塞后导致肛腺感染,形成肛周脓肿。Objective:To clarify the role of androgen and its receptors in perianal abscess. Methods:From May 2017 to May 2018, 196 patients with perianal abscess, anal fistula and mixed hemorrhoids were enrolled in the general surgery department of Qingdao Municipal Hospital. They were divided into study group, 112 patients with perianal abscess and anal fistula, 89 males and 23 females, with an average age of 37.75(+11.2 years), with a median age of 34 years (18~63 years). The control group consisted of 84 patients, including 43 males and 41 females. The median age was 37 years (18~63 years). The serum androgen and estrogen levels and the expression of androgen receptors in anal glands and anal glands in the dentate line of anal canal were observed. Results:The level of serum androgen in male study group was lower than that in control group at all ages(P<0.05);in female study group, the level of serum androgen was lower than that in control group at 30-39 years old(P<0.05), but there was no significant difference between 18~29 years old and 40~45 years old(P>0.05);in male and female study group, the level of estrogen in each stage was not significantly higher than that in control group(P>0.05). The positive rate of anal gland in dentate line area was significantly less than that in control group(P<0.05), and the number of androgen receptors in anal gland in study group was significantly more than that in control group(P<0.05). Conclusion:The level of serum androgen in male patients with perianal abscess was lower than that in normal persons, but there was no significant increase or decrease in female patients. The positive rate of anal gland near dentate line in patients with perianal abscess was lower than that in normal persons, and the level of androgen receptor in anal gland increased compensatively.There is a blood androgen-androgen receptor-anal gland reaction axis in the body.
