养血柔肝针法对卒中后抑郁患者血浆5-HT的影响

摘　　要：目的基于血浆5-HT变化,观察养血柔肝针法治疗卒中后抑郁症的取效机制.方法将60例卒中后抑郁症患者按照随机数字表法随机分为观察组和对照组.观察组采用养血柔肝针法,对照组采用口服盐酸氟西汀.对两组患者的综合疗效、治疗前后汉密尔顿抑郁量表(HAMD)评分、抑郁自评量表(SDS)评分、日常生活能力Barthel指数(BI)评分、中国脑卒中临床神经功能缺损程度评分表(CSS)评价神经功能及血浆中5-羟色胺(5-HT)含量进行对比观察.结果观察组总有效率为90.0%,对照组为66.7%,差异具有统计学意义(P<0.05);两组治疗后HAMD、SDS、CSS评分均较治疗前降低(P<0.05),BI、血浆5-HT含量均较治疗前升高(P<0.05),且组间比较差异有统计学意义(P<0.05).结论养血柔肝针法可明显改善卒中后抑郁患者的抑郁状态、神经功能缺损程度、日常生活功能,且能明显提高患者血浆中5-HT含量,其疗效优于传统药物治疗.Objective To observe the action mechanism of Yang Xue Rou Gan (blood-nourishing and liver-soothing) needling method in treating post-stroke depression based on the change in plasma 5-hydroxytryptamine (5-HT). Method Sixty post-stroke depression patients were randomized into an observation group and a control group by random number table method. The observation group was intervened by Yang Xue Rou Gan needling method, while the control group was intervened by oral administration of fluoxetine hydrochloride. The comprehensive efficacies of the two groups were compared, as well as the scores of Hamilton Depression Scale (HAMD), Self-rating Depression Scale (SDS), Barthel Index (BI) and China Stroke Scale (CSS), and the content of plasma 5-HT before and after treatment. Result The total effective rate was 90.0% in the observation group versus 66.7% in the control group, and the difference was statistically significant (P<0.05);in both groups, the scores of HAMD, SDS and CSS decreased (P <0.05), and the BI score and the 5-HT content increased (P<0.05) after treatment, and the between-group differences were statistically significant (P<0.05). Conclusion Yang Xue Rou Gan needling method can markedly improve the depressive state, neurological deficit degree and activities of daily living, and up-regulate the content of plasma 5-HT in post-stroke patients. It can produce more significant efficacy than conventional medication.
