摘  要:目的观察经皮穴位电刺激联合康复训练对卒中后神经源性膀胱的尿动力学的影响.方法选取卒中后神经源性膀胱患者84例,随机分为治疗组和对照组,每组42例.对照组采用常规康复训练,治疗组在对照组治疗的基础上,采用经皮穴位电刺激治疗.记录并对比分析两组临床疗效、排尿日记(日均排尿次数、日均单次排尿量、日均尿失禁次数)、尿动力学(最大膀胱容量、最大尿流速率和残余尿量)等水平.结果两组治疗后日均排尿次数、日均尿失禁次数均较治疗前明显降低(P<0.05),日均单次排尿量均较治疗前明显升高(P<0.05);治疗组治疗后日均排尿次数、日均尿失禁次数均明显低于对照组(P<0.05),日均单次排尿量明显高于对照组(P<0.05);两组治疗后最大膀胱容量、最大尿流速率、膀胱排尿压力均较同组治疗前升高(P<0.05);治疗组治疗后最大膀胱容量、最大尿流速率、膀胱排尿压力均高于对照组(P<0.05);治疗组临床疗效明显优于对照组(P<0.05).结论经皮穴位电刺激联合康复训练治疗卒中后神经源性膀胱,能改善尿动力学指标,促进膀胱功能恢复.Objective To observe the effect of transcutaneous electrical acupoint stimulation (TEAS) plus rehabilitation training on urodynamics in post-stroke neurogenic bladder (NB). Method Eighty-four patients with post-stroke neurogenic bladder were randomized into a treatment group and a control group, with 42 cases in each group. The control group was intervened by conventional rehabilitation training, while the treatment group was additionally given TEAS. The clinical efficacies of the two groups were compared, and the voiding diary (daily voiding frequency, average urinary volume and daily urinary incontinence frequency) and urodynamic indexes (maximum bladder capacity, maximum urinary flow rate and residual urine volume) were recorded and compared. Result The daily voiding frequency and daily urinary incontinence frequency declined significantly after treatment in the two groups (P<0.05), and the average urinary volume increased significantly (P<0.05);after treatment, the daily voiding frequency and daily urinary incontinence frequency in the treatment group were significantly lower than those in the control group (P<0.05), and the average urinary volume in the treatment group was larger than that in the control group (P<0.05);the maximum bladder capacity, maximum urinary flow rate and bladder voiding pressure increased after treatment in the two groups (P<0.05);the maximum bladder capacity, maximum urinary flow rate and bladder voiding pressure in the treatment group were higher than those in the control group (P<0.05);the clinical efficacy in the treatment group was more significant than that in the control group (P<0.05). Conclusion TEAS plus rehabilitation training can improve urodynamic indexes and boost the recovery of bladder function in treating post-stroke NB.
