摘 要:目的:通过对便携式变压吸附(pressure swing adsorption,PSA)制氧机进行高原适应性试验和应用效果的初步评价,以期为高原人群提供更科学有效的富氧干预手段。方法:采用低压舱分别模拟3 000、4 000、5 000 m 3个海拔高度,在各海拔高度对制氧机的富氧气体氧体积分数和供气量进行测试,同时采集8名被试人员在每个海拔高度使用制氧机富氧前后的血氧饱和度(SpO2)和心率(HR)数据,并赴海拔3 800 m地区采集另外8名被试人员使用制氧机富氧前后的SpO2和HR数据,通过对比使用该制氧机富氧前后被试人员SpO2和HR的变化对其应用效果进行初步评价。结果:该制氧机的富氧气体氧体积分数和供气量会随着海拔的增高有一定的下降(P<0.05),但是在海拔5 000 m高度时富氧气体氧体积分数仍能维持在88%,供气量高于36 ml/次,且在固定海拔高度时其富氧气体氧体积分数和供气量波动不大;在模拟高原环境和高原实地使用该制氧机富氧后被试人员的SpO2显著升高(P<0.05),HR显著下降(P<0.05);在海拔3 800 m,富氧3 min对SpO2的提高效果可以至少持续到停止富氧后10 min(P<0.05),HR在停止富氧后1 min内迅速升至富氧前水平。结论:便携式PSA制氧机性能稳定,能够有效缓解急进高原人群缺氧状态,对于保障进驻高原人群的身体健康具有十分重要的意义。Objective To conduct plateau adaptability test and preliminary evaluation of the application effect of the portable PSA oxygen generator so as to provide scientific and effective means of oxygen-enriched intervention for the plateau population. Methods Acute hypobaric hypoxia environment at 3 000, 4 000 and 5 000 m altitudes were simulated by the hypobaric chamber. The oxygen concentration and flow rate of the oxygen generator were tested at each altitude. At the same time, data of SpO2 and HR of 8 subjects were collected before and after using oxygen generator at each altitude. Data of the other 8 subjects were collected at the 3 800 m plateau. The application effect of the oxygen generator was preliminarily evaluated by comparing SpO2 and HR changes. Results The oxygen concentration and flow rate of the oxygen generator decreased with the increase of altitude;while the oxygen concentration could still be maintained at 88%, and the oxygen-enriched air was higher than 36 ml/time at the altitude of 5 000 m. Moreover, the oxygen concentration and air supply were stable at the fixed altitude. After using the oxygen generator, the average SpO2 of subjects were significantly increased (P<0.05) and the average HR were statistically decreased (P<0.05) in the simulated plateau environment and plateau. At altitude of 3 800 m, the gain on SpO2 could be maintained at least 10 min after 3 min oxygen inhalation (P<0.05). However, the sustained gain of HR had no statistical significance. Conclusion The portable PSA oxygen generator has stable performances, and can effectively relieve the hypoxia of the population after rush entry into the plateau, which is of great significance for guaranteeing the health of the people at the plateau.
