摘  要:目的探讨二维斑点追踪超声心动图技术(STE)评估不同病程川崎病(KD)患儿左心室心肌运动功能的价值。方法选取2017年1月~2017年12月在我院儿科就诊的川崎病患儿47例为研究对象,根据患儿病程情况分为急性期(n=11)、恢复期(n=12)及慢性期(n=24),以上3组为观察组,并选取同期55例健康儿童为对照组,所有儿童均采用STE对左心室心肌运动功能进行评估。结果观察组三期患儿GLS、GCS、GCSR指标均低于对照组,差异有统计学意义(均P<0.05);急性期LVRWT指标高于恢复期和慢性期(P<0.05);恢复期二尖瓣环近游离壁舒张早期心肌运动速度高于急性期和慢性期,急性期与慢性期近室间隔收缩期速度均明显低于对照组(均P<0.05)。结论KD患儿在不同阶段中心肌运动功能有不同程度的损伤,STE对评估KD患儿左心室心肌运动功能有一定的临床价值。ObjectiveTo investigate the value of two-dimensional speckle tracking echocardiography (STE)in evaluating left ventricular myocardial motor function of Kawasaki disease (KD) children at different phases. Methods47 KD children treated in the pediatrics department of our hospital from January 2017 to December 2017 were selected and divided into acutephase group ( n =11), convalescent phase group ( n =12) and chronic phase group ( n =24). 55 healthy children were set as control group. The left ventricular myocardial motor function of 102 children was assessed by STE. ResultsThe GLS, GCS and GCSR were lowest in control group ( P <0.05). The GLS was lower in the acute phase group and chronic phase group than in the control group ( P <0.05). The decreasing degree of GCSR in the convalescent phase group was more significant than in the chronic phase group ( P <0.05). The LVRWT of the acute phase group was higher than that of the convalescent phase group and chronic phase group ( P <0.05). The lateral wall myocardial early diastolic velocity was higher in the convalescent phase group than in the acute phase group and chronic phase group ( P <0.05), as well as higher in the control group than in the acute phase group ( P <0.05). ConclusionDifferent degrees of left ventricular myocardial motor function deformation are existed in KD children, so the two-dimensional STE dose have certain evaluative value for the KD children.
