右美托咪定对老年全麻腹腔镜患者应激激素及血流动力学状态的影响研究

摘　　要：目的:观察右美托咪定对老年全麻腹腔镜患者应激激素及血流动力学状态的影响。方法:选取2017年1月-2018年1月本院86例老年全麻腹腔镜手术患者。按照随机数字表法将其分为对照组(未应用右美托咪定)和观察组(应用右美托咪定),各43例。比较两组诱导前、插管即刻、拔管即刻及拔管后5 min的血流动力学指标及应激激素水平、呛咳及躁动评分。结果:诱导前,两组血流动力学指标及应激激素水平比较,差异均无统计学意义(P>0.05);插管即刻、拔管即刻及拔管后5 min,观察组血流动力学指标及应激激素水平均低于对照组,差异均有统计学意义(P<0.05);观察组呛咳及躁动程度均优于对照组(P<0.05)。结论:右美托咪定对老年全麻腹腔镜患者应激激素及血流动力学状态的影响相对较好,在本类老年患者中具有更高的应用价值。Objective:To observe the influence of Dexmedetomidine for the stress hormones and hemodynamic state of elderly patients with laparoscopic surgery of general anesthesia.Method:A total of86 elderly patients with laparoscopic surgery of general anesthesia in our hospital from January 2017 to January2018 were selected.According to the random number table method,they were divided into control group(without Dexmedetomidine)and observation group(with Dexmedetomidine),43 cases in each group.The hemodynamic indexes and stress hormone levels before induction,immediately intubation,immediately extubation and 5 minutes after extubation,cough and restlessness scores between two groups were compared.Result:Before induction,the hemodynamic indexes and stress hormones levels of two groups were compared,the differences were not statistically significant(P>0.05).Immediately intubation,immediately extubation and 5 minutes after extubation,the hemodynamic indexes and stress hormones levels of observation group were lower than those of control group,the differences were statistically significant(P<0.05).The degree of cough and restlessness of observation group were better than those of control group(P<0.05).Conclusion:Dexmedetomidine has a relatively good effect on stress hormones and hemodynamics in elderly patients with laparoscopic surgery of general anesthesia,and has a higher application value in this kind of elderly patients.
