摘　　要：目的:探究经皮肾镜碎石取石术患者术中采取不同体位对其手术及疗效的影响。方法:选取2018年1-10月本院收治的84例行经皮肾镜碎石取石术治疗的患者为研究对象,根据完全随机法将其分为对照组与观察组,每组42例。对照组经皮肾镜碎石取石术术中采取俯卧位,观察组经皮肾镜碎石取石术术中采取斜仰卧位。对两组手术时间、通道建立时间、术中出血量、术中舒适度、结石一次清除率、术后并发症及住院时间等指标进行比较。结果:两组术中出血量、通道建立时间、住院时间、结石一次清除率、术后并发症比较,差异均无统计学意义(P>0.05);观察组手术时间明显短于对照组(P<0.05);观察组术中舒适度明显高于对照组(P<0.05)。结论:经皮肾镜碎石取石术中患者取斜仰卧体位更有利于手术的顺利进行。体位简化进而缩短手术时间,提升患者术中舒适度,减少患者反复摆体位所浪费的时间。一个体位可以同时处理双侧输尿管结石等,能促进患者快速康复,因此临床中可针对该种体位及手术方式进行推广。Objective:To explore the effects of different positions in percutaneous nephrolithotripsy on the operation and curative effect.Method:84 patients underwent percutaneous nephrolithotomy and were admitted to our hospital from January to October 2018 were selected.They were divided into control group and observation group according to the completely random method,42 cases in each group.The control group was given prone position during the percutaneous nephrolithotomy,the observation group was in recumbent supine position during the percutaneous nephrolithotomy.The operation time,channel establishment time,intraoperative blood loss,intraoperative comfort,primary calculus clearance rate,postoperative complications and hospital stays in the two groups were analyzed and compared.Result:There were no significant differences between the two groups in intraoperative blood loss,channel establishment time,hospital stays,primary calculus clearance rate and postoperative complications(P>0.05).The operation time of the observation group was significantly lower than that of the control group(P<0.05).The intraoperative comfort of the observation group was significantly higher than that of the control group(P<0.05).Conclusion:Recumbent supine position is more conducive to the smooth operation for patient percutaneous nephrolithotomy.The simplification of postures can shorten the operation time,improve the patients’intraoperative comfort,reduces the time of repeatedly turning over the patients’body in the surgery.One position can be used for simultaneous treatment of bilateral ureteral stones etc.It can promote rapid recovery of patients.Thus this position and surgical methods can be promoted clinically.
