健脾缩腺颗粒对腺样体肥大患儿Th17/Treg的影响研究

摘　　要：目的:探讨给予腺样体肥大患儿健脾缩腺颗粒治疗的临床效果及对外周血辅助性T细胞17(Th17)/调节性T细胞(Treg)比值产生的影响.方法:选取2018年4-12月本院收治的80例腺样体肥大患儿,按用药方式不同分为三组,中药组(n=26)给予健脾缩腺颗粒治疗,西药组(n=26)给常规西药治疗,联合组(n=28)给予健脾缩腺颗粒+西药治疗.观察比较三组患儿症状体征、生活质量、Th17/Treg比值等情况.结果:治疗后12周,联合组各症状积分、腺样体体积积分均显著低于其他两组,OSA-18量表4个维度积分及总分均显著高于其他两组,差异均有统计学意义(P<0.05);西药组、中药组上述指标比较,差异均无统计学意义(P>0.05).三组治疗期间均无患儿出现明显不良反应.治疗后12周,联合组Th17显著高于其他两组,Th17/Treg比值则显著更低(P<0.05).结论:在常规西药治疗基础上再给予腺样体肥大患儿健脾缩腺颗粒治疗,可明显提高症状、体征缓解效果,降低Th17/Treg比值,有效调节免疫平衡,改善患儿生活质量.Objective:To evaluate the clinical effect of Jianpi Suoxian Granule on children with adenoid hypertrophy and its effect on the ratio of helper T cells 17(Th17)/regulatory T cells(Treg)in peripheral blood. Method:Eighty children with adenoidal hypertrophy admitted to our hospital from April 2018 to December 2018 were selected,they were divided into three groups according to the different ways of using drugs,the traditional Chinese medicine group(n=26)was given Jianpi Suoxian Granule treatment,the western medicine group (n=26)was given conventional western medicine treatment,and the combined group(n=28)was given Jianpi Suoxian Granule + western medicine treatment.The symptoms and signs,quality of life,and Th17/Treg ratio of the three groups were observed and compared.Result:12 weeks after treatment,the symptom scores and adenoid volume scores of the combined group were significantly lower than those of the other two groups,and the four dimension scores and total scores of OSA-18 scale were significantly higher than those of the other two groups, with statistically significant differences(P<0.05);comparison of the above indicators between the western medicine group and the Chinese medicine group showed no statistically significant differences(P>0.05).There were no obvious adverse reactions during the treatment among the three groups.At 12 weeks after treatment,Th17 of the combined group was significantly higher than those of the other two groups,and the Th17/Treg ratio was significantly lower(P<0.05).Conclusion:On the basis of conventional western medicine treatment,Jianpi Suoxian Granule treatment for children with adenoid hypertrophy can significantly improve the relief effect of symptoms and signs,reduce the Th17/Treg ratio,effectively regulate the immune balance,and improve the quality of life of children.
