快速康复护理联合脐敷对腹式全子宫切除术后胃肠功能恢复的有效性分析

摘　　要：目的:分析快速康复护理联合脐敷对腹式全子宫切除术后胃肠功能恢复的有效性.方法:选取2014年9月-2015年3月于本院行腹式全子宫切除术的患者146例.按照不同干预方法将其分为对照组和研究组,各73例.对照组应用中药脐敷及常规护理,研究组应用中药脐敷联合快速康复护理.比较两组术后相关胃肠功能指标恢复时间、住院时间、相关盆底功能指标恢复情况、临床疗效、生活质量.结果:研究组术后相关胃肠功能指标恢复时间及住院时间均短于对照组(P<0.05);研究组盆底收缩压、肌力均高于对照组,盆底收缩持续时间长于对照组,差异均有统计学意义(P<0.05);研究组总有效率为98.6%,高于对照组的87.7%(P<0.05);研究组生活质量各维度评分均低于对照组(P<0.05).结论:在给予腹式全子宫切除术患者应用脐敷的过程当中联合应用快速康复护理,可显著缩短患者术后各项胃肠功能指标恢复时间及住院时间;同时还能显著提高患者的生活质量,促使患者的各项盆底功能得到更快恢复.Objective:To analyze the effectiveness of rapid rehabilitation nursing combined with umbilical compress in the recovery of gastrointestinal function after abdominal total hysterectomy.Method:A total of 146 patients underwent abdominal total hysterectomy in our hospital from September 2014 to March 2015 were selected. According to different intervention methods,they were divided into control group and study group,73 cases in each group.The control group was given umbilical application of traditional Chinese medicine and routine nursing, while study group was given umbilical application of traditional Chinese medicine combined with rapid rehabilitation nursing.The recovery time of gastrointestinal function,hospitalization time,recovery of pelvic floor function, clinical efficacy and quality of life between two groups were compared.Result:The recovery time of gastrointestinal function indexes and hospitalization time in study group were shorter than those of control group(P<0.05).The pelvic floor systolic pressure and muscle strength of study group were higher than those of control group,and the duration of pelvic floor contraction was longer than that of control group,the differences were statistically significant (P<0.05).The total effective rate was 98.6% in study group,which was higher than 87.7% of control group (P<0.05).The scores of each dimension of QOL in study group were lower than those of control group(P<0.05). Conclusion:In the process of applying umbilical compress to patients undergoing abdominal hysterectomy, combined application of rapid rehabilitation nursing can significantly shorten the recovery time of gastrointestinal function indicators and hospitalization time,at the same time,it can significantly improve the quality of life of patients and promote faster recovery of pelvic floor functions.
