PFNA与DHS治疗老年股骨粗隆间骨折的临床效果及对术后恢复情况的对比研究

摘　　要：目的:比较股骨近端抗旋转刀片髓内钉内固定术(PFNA)与动力髋加压螺钉内固定术(DHS)治疗老年股骨粗隆间骨折的临床效果及对患者术后恢复情况的影响.方法:选取2017年1月-2018年9月本院收治的需手术治疗的120例老年股骨粗隆间骨折患者为研究对象,按照手术方式分为试验组和对照组,各60例.试验组采用PFNA治疗,对照组采用DHS治疗.比较两组患者临床治疗效果、骨痂出现时间、骨痂评分、骨折线出现时间及VAS评分.结果:试验组的优良率为90.00%,明显高于对照组的66.67%,差异有统计学意义(P<0.05);试验组的骨痂出现时间、骨折线出现时间均短于对照组,差异均有统计学意义(P<0.05);试验组术后1、2个月的骨痂评分均高于对照组,差异均有统计学意义(P<0.05);试验组术后1、2、3个月的VAS评分均低于对照组,差异均有统计学意义(P<0.05).结论:PFNA对于治疗老年股骨粗隆间骨折,效果好、利于骨折的愈合,可有效降低患者术后疼痛.Objective:To compare the clinical effect of proximal femur anti-rotating blade internal fixation with intramedullary nail(PFNA)and dynamic hip compression screw internal fixation(DHS)in the treatment of intertrochanteric femoral fracture in the elderly and the effect on postoperative recovery of patients. Method:A total of 120 elderly patients with intertrochanteric fractures admitted to our hospital from January 2017 to September 2018 and requiring surgical treatment were selected as research objects,according to the operation method,patients were divided into experimental group and control group,60 cases in each group.PFNA was used in the experimental group and DHS was used in the control group.The clinical treatment effect,callus occurrence time,callus score,fracture line occurrence time and VAS score were compared between the two groups. Result:The excellent and good rate of the experimental group was 90.00%,which was significantly higher than 66.67% of the control group,the difference was statistically significant(P<0.05).The occurrence time of callus and fracture line in the experimental group were shorter than those in the control group,with statistically significant differences(P<0.05).Callus scores in the experimental group at 1 and 2 months after operation were higher than those in the control group,with statistically significant differences(P<0.05).VAS scores of the experimental group at 1,2 and 3 months after operation were lower than those of the control group, with statistically significant differences(P<0.05).Conclusion:PFNA has a good effect on the treatment of intertrochanteric fracture in the elderly,which is conducive to fracture healing and can effectively reduce postoperative pain in patients.
