摘 要:目的观察超声造影应用于甲状腺结节良恶性定性诊断中的临床价值并进行分析.方法选择2016年1月—2018年12月在该院接受检查的153例甲状腺结节患者作为研究对象,先后行常规超声、超声造影和病理诊断.对比常规超声与超声造影的图像特点、检查结果及与病理学检查的一致性、诊断准确率、灵敏度和特异性.结果超声造影与病理诊断结果一致性高于常规超声与病理结果诊断一致性(kappa:0.791 vs 0.328)(P<0.05);超声造影诊断准确率、灵敏度和特异性均高于常规超声[准确率:91.50%vs 69.28%(x^2=23.966,P<0.05;灵敏度:94.50%vs 70.64%[(x^2=21.545,P<0.05);特异性:84.09%vs 65.91%(x^2=3.879,P<0.05)].结论超声造影应用于甲状腺结节良恶性定性诊断中的临床价值较常规超声高.在临床工作中可以借助常规超声进行初步判断,对于难以鉴别的结节可以联合超声造影对结节性质进一步判断,提高甲状腺结节检出率.Objective To observe the clinical value of contrast-enhanced ultrasound in the qualitative diagnosis of benign and malignant thyroid nodules. Methods A total of 153 patients with thyroid nodules who were examined in our hospital from January 2016 to December 2018 were enrolled in the study. Routine ultrasound, contrast-enhanced ultrasound and pathological diagnosis were performed. The image characteristics, examination results and consistency with pathological examination, diagnostic accuracy, sensitivity and specificity of conventional ultrasound and contrast ultrasound were compared. Results The consistency of contrast-enhanced ultrasound and pathological diagnosis was higher than that of conventional ultrasound and pathological diagnosis (kappa:0.791 vs 0.328)(P<0.05). The accuracy, sensitivity and specificity of contrast-enhanced ultrasound imaging were higher than those of conventional ultrasound [accuracy rate: 91.50% vs 69.28%(χ^2=23.966, P<0.05);sensitivity: 94.50% vs 70.64%(χ^2=21.545, P<0.05);specificity: 84.09% vs 65.91%(χ^2=3.879, P<0.05)]. Conclusion The clinical value of contrast-enhanced ultrasound in the qualitative diagnosis of benign and malignant thyroid nodules is higher than that of conventional ultrasound. In clinical work, preliminary judgment can be made by means of conventional ultrasound. For the nodules which are difficult to identify, the combination of contrast-enhanced ultrasound can further judge the nature of nodules and improve the detection rate of thyroid nodules.
