益气通络法配合手术治疗腰椎间盘突出症的临床研究

摘　　要：目的:探讨益气通络法组方配合腰椎后路椎间融合术(PLIF)治疗寒湿痹阻型腰椎间盘突出症的临床效果及对外周血肿瘤坏死因子-α(TNF-α)和白细胞介素-1β(IL-1β)水平的影响,进一步探讨该方法治疗机制。方法:选择2016年5月-2017年10月本院收治的寒湿痹阻型腰椎间盘突出症患者80例,随机分为两组,治疗组40例采用益气通络法组方配合PLIF手术治疗,对照组40例采用PLIF手术治疗。观测记录术前,术后1周、1个月日常生活能力评分(ADL)、VAS评分(VAS)、血清TNF-α和IL-1β水平及两组住院天数。结果:两组患者均获得满意临床疗效,但治疗组优于对照组,差异有统计学意义(P<0.05)。结论:寒湿痹阻型腰椎间盘突出症PLIF术后配合益气通络法治疗,可降低术后外周血TNF-α和IL-1β水平,减轻术后疼痛,提高术后早期疗效,促进患者快速康复。Objective:To explore the clinical efficacy of Boosting Qi and Dredging Collateral combined with PLIF in treating cold-dampness-blocking type lumbar disc herniation,and its effects on peripheral blood tumor necrosis factor-α(TNF-α)and interleukin-1 β(IL-1β).The therapeutic mechanism of this method was further discussed.Method:From May 2016 to October 2017,80 patients with cold-dampness-blocking type lumbar disc herniation were randomly divided into two groups.40 cases in the treatment group were treated with Boosting Qi and Dredging Collateral combined with PLIF operation,40 patients in the control group were treated with PLIF operation.Preoperative and one week,one month after operation,activity of daily life(ADL),VAS scores(VAS),serum TNF-α,IL-1β levels and hospitalization days of two groups were recorded.Result:Both two groups achieved satisfactory clinical efficacy,but the treatment group were superior to the control group,and the differences were statistically significant(P<0.05).Conclusion:Boosting Qi and Dredging Collateral combined with PLIF in treating cold-dampness-blocking type lumbar disc herniation can reduce the serum TNF-α,IL-1β levels of postoperation,alleviate postoperative pain,improve the early postoperative curative effect and promote the patients’ rapid rehabilitation.
