张力平衡针法对卒中后痉挛瘫痪患者表面肌电的短期影响

摘　　要：目的:观察张力平衡针法对卒中后痉挛瘫痪病人肱二头肌肌张力的即时及短期影响。方法:将符合标准的脑卒中后恢复期患者总共52例,随机分成张力平衡组(26例)和常规针刺组(26例),治疗上分别采用张力平衡针法和常规针刺,1次/d,观察3天。分别于第1天、第2天、第3天治疗前及治疗后采用表面肌电分析仪测定患者患侧肱二头肌的均方根值(RMS),并用改良Ashworth分级(MAS)评价患肢肌张力情况。结果:针刺后,张力平衡组和常规针刺组病人的表面肌电均方根值(RMS)均较治疗前降低(P<0.05),且张力平衡组优于常规针刺组,而MAS未见明显变化。结论:①张力平衡针法可在短时间内改善卒中后患者的痉挛状态,疗效优于常规针刺;②较之MAS,表面肌电均方根值(RMS)更能灵敏反应肌张力的变化。Objeetive:To observe short-term and instant influence of tension balance acupuncture on stroke survivors’ s EMG of museulus biceps braehii.Methods:Divide a total of 52 patients with spastic state of stroke into the tension balance acupuncture group(26 cases)and the ordinary acupuncture group(26 cases).Patients of the tension balance acupuncture group were treated with tension balance acupuncture,while patients of the orcdinary acupuncture group were given ordinary acupuncture,both groups were given acupuncture once a day for 3 days.The Modified Ashworth Scale and RMS value of sEMG were observed before and after acupuncture on the 1 st day,the 2 nd day,and the 3 rd day.Results:Compared with before treatment,the RMS values of biceps braohii in the both groups decreased in different degrees after treatment(P<0.05),and the decreases of the tension balance acupuncture group was more remarkable than those of the ordinary acupuncture group,while the MAS didn’t change.Conelusion:(1)Tension balance acupuncture can release the stroke spastic state in short-time,which is more effective than the ordinary acupuncture.(2)sEMG is more sensitive than the MAS in demonstrating the level of distonia.
