宁夏盐池县农村中老年人慢性病患病情况及其公平性分析

摘　　要：目的了解宁夏盐池县农村中老年人的慢性病患病情况,评价不同经济收入中老年人慢性病患病公平性,分析慢性病患病不公平的影响因素。方法利用"创新支付制度,提高卫生效益"试点项目中中老年人的调查数据,以患病率描述慢性病的患病情况,以集中指数为公平性评价指标,采用集中指数分解法分析影响慢性病患病不公平的因素。结果宁夏盐池县基线调查(2009年)及2011年、2015年追踪调查的中老年人慢性病患病率分别为34.08%(590/1 731)、22.84%(413/1 808)和44.83%(976/2 177);3次调查的中老年人慢性病患病率的集中指数均为负值,分别是-0.023、-0.029、-0.013;各因素对慢性病患病不公平的贡献中,年龄、文化程度、性别是导致慢性病患病不公平的主要影响因素。结论宁夏盐池县中老年人慢性病患病率整体水平较低,慢性病患病的公平性总体状况较好,年龄是对慢性病患病不公平贡献最大的因素,应加强对低收入人群、老年人群及女性等弱势群体健康的关注,普及健康保健知识,提高农村居民健康公平性,改善其健康状况。Objective To investigate the prevalence of chronic diseases among the mid-aged and elderly in Yanchi County of Ningxia,and to evaluate its equity between different income groups,and to analyze the influencing factors of chronic diseases.Methods The data were collected from the Ningxia "innovation payment system,improve the health benefit" project,and analyzed with the prevalence of chronic diseases,the concentration index(CI)and the decomposition method.Results The prevalence of chronic diseases among the middle-aged and elderly were 34.08%(590/1 731),22.84%(413/1 808),44.83%(976/2 177),respectively in the baseline survey and in the two follow-up surveys,CIs were-0.023,-0.029,-0.013,respectively.The influencing factors increased the inequity of chronic diseases prevalence were age,literacy,females.Conclusion The prevalence of chronic diseases is relatively lower among mid-aged and elderly in rural areas in Yanchi County of Ningxia.The overall situation on equities of the prevalence is good,and age had the highest contribution to the inequity.It suggests to improve the accessibility of health services among females,poorer and older population,popularization of health knowledge,and to improve the health equity of rural residents for promoting health.
