红河州医疗机构2010—2017年HIV抗体检测及病例发现情况分析

摘　　要：目的通过分析2010-2017年红河州医疗机构艾滋病病毒(HIV)抗体检测及病例发现情况,为红河州医疗机构制定针对性的艾滋病防控措施提供参考。方法收集2010-2017年红河州医疗机构上报中国疾病预防控制系统艾滋病综合防治信息系统数据,并对数据进行统计分析。结果红河州医疗机构开展HIV抗体检测人次数从2010年15万人次增至2017年的110万人次,占全州的60.00%;新发现报告病例从2010年的32.86%增至2017年的62.85%;粗检出率从2010年的42.86/万降至2017年的12.57/万。2010-2017年医疗机构新发现报告艾滋病病例,50岁以上年龄组占比从2010年20.84%增至2017年的41.03%;农民占比从2010年的59.72%增至2017年的80.56%。异性性传播、同性性传播呈逐年增长趋势。接受CD4+T淋巴细胞检测比例从2010年的66.72%增至2017年的92.54%,8年来新发现报告病例CD4+T淋巴细胞检测结果<350个/u L占58.36%。结论红河州医疗机构检测发现艾滋病病例是全州发现艾滋病病例报告的主要来源,且发现的病例CD4+T淋巴细胞水平较低。应在医疗机构体检人群、门诊就诊者等人群中加强艾滋病监测检测力度,针对发现的艾滋病病毒感染者采取有效干预措施,做到早发现、早诊断和早治疗,减少HIV的二代传播。OBJECTIVE To analyze HIV testing and cases finding in medical institutions in Honghe Prefecture from 2010 to 2017 inorder to provide references for medical institutions to formulate targeted AIDS prevention and control. METHODS The data of AIDS comprehensive prevention and control information system reported by medical institutions in Honghe Prefecture from 2010 to 2017 were collected and analyzed. RESULTS From 2010 to 2017,the number of HIV screening tests offered in medical institutions accounts for 60.00% of total number in Honghe Prefecture,increasing form 150 thousand to 1.1 million. New reported HIV cases increased from 32.86% in 2010 to62.85% in 2017. HIV positive coarse rate decreased from 42.86/10 thousand to 12.57/10 thousand. The proportion of over 50 years old group increased from 20.84% to 41.03%;the proportion of famer increased from 59.72% to 80.56%. Heterosexual transmission and homosexual transmission increased. The proportion of CD4 + T increased from 66.72% to 92.54%. The proportion of CD4 + T lymphocyte <350/ul was 58.36% in 8 years. CONCLUSION Medical institutions are main HIV/AIDS cases detection channel,CD4+T lymphocyte levels are lower when the cases are identified. HIV monitoring and testing among physical examination in medical institutions,outpatients and other groups should be strengthened. It is necessary to take effective intervention among HIV/AIDS which it can be early detection,early diagnosis and early treatment,so as to reduce the secondary transmission.
