摘　　要：目的监测和分析江苏省苏州地区儿童肺炎链球菌(SP)的血清型分布及药物敏感情况,为临床抗生素应用及疫苗接种提供科学依据。方法收集苏州大学附属儿童医院2013-2015年就诊儿童中分离的SP 250株,血清分型采用荚膜肿胀实验,药物敏感性分析采用最小抑菌浓度法(MIC)。结果侵袭性菌株常见血清型为6B、14、19A和19F,非侵袭性菌株为19F、23F、6B、6A、19A和14。侵袭性菌株的PCV13血清型覆盖率(91.8%)和耐药率普遍高于非侵袭性菌株,疫苗血清型菌株的耐药率普遍高于非疫苗血清型菌株。血清型19F数量及耐药率均高于其他血清型。结论苏州市儿童PCV13疫苗血清型覆盖率高,提示疫苗接种可有效预防肺炎链球菌感染,对耐药菌株的传播也有积极的控制作用。OBJECTIVE To provide baseline data for antibiotic use and vaccination,to analyze the serotype distribution and antimicrobial resistance patterns of streptococcus pneumonia in Suzhou Ragion of Jiangsu Province. METHODS 250 strains of streptococcus pneumoniae isolated from children admitted in Soochow University Affiliated Children’s Hospital(SCH)from 2013 to 2015 were collected. The serotypes of pneumococcal isolates were detected by the Quellung reaction. Minimum inhibitory concentration method(MIC)was used to detect antimicrobial resistance. RESULTS The most common serotypes of invasive isolates were 6 B,14,19 A and 19 F. For non-invasive isolates,the most common serotypes were 19 F,23 F,6 B,6 A,19 A and 14. The PCV13 serotype coverage rate(91.8%)and drug resistance rate of invasive strains were generally higher than non-invasive strains. What’s more,the drug resistance rate of vaccine serotype was generally higher than that of non-vaccine serotype. CONCLUSION The high serotype coverage rate of PCV13 for children in Suzhou indicates that vaccination could effectively prevent streptococcus pneumoniae infection and control the spread of drug-resistant strains.
