振动正压呼气排痰设备在慢性阻塞性肺疾病康复护理中的应用

摘　　要：目的观察振动正压呼气(oPEP)排痰设备在慢性阻塞性肺疾病(COPD)患者康复护理中的应用效果。方法 80例COPD患者随机分为对照组和研究组,各40例。对照组给予常规康复护理,研究组在对照组治疗的基础上联合应用oPEP排痰设备,比较2组肺功能及动脉血气指标,并记录平均24 h排痰量及痰液好转天数。结果实施护理干预后,研究组一秒用力呼气容积(FEV1)和动脉血氧分压(PaO2)水平高于对照组,而用力肺活量(FVC)和动脉血二氧化碳分压(PaCO2)水平低于对照组,差异有统计学意义(P<0.05)。研究组平均24 h排痰量高于对照组,而痰液性状好转天数短于对照组,差异有统计学意义(P<0.05)。结论运用oPEP设备排痰,能够提高COPD康复护理效果,改善患者的肺功能,促进痰液排出。Objective To investigate the effect of the oscillatory positive expiratory pressure(oPEP)device in rehabilitation care of patients with chronic obstructive pulmonary disease(COPD). Methods Totally 80 patients with COPD were randomly divided into the control group and the study group, with 40 cases in each group. All patients received routine rehabilitation care, and the oPEP device was used for clearance of pulmonary sputum in the study group. The pulmonary function and indexes of arterial blood gas were compared between two groups. The daily amount of sputum and days to sputum properties improved were observed. Results Patients in the study group achieved a higher forced expiratory volume in one second(FEV1)and arterial partial pressure of oxygen(PaO2)compared with those in the control group(P<0.05). The forced vital capacity(FVC)and arterial partial pressure of carbon dioxide(PaCO2)in the study group were lower than those in the control group(P<0.05), Patients in the study group had a higher daily amount of sputum clearance and fewer days to sputum properties improved compared with those in the control group(P<0.05). Conclusion The application of oPEP device is effective for clearance of pulmonary sputum and rehabilitation of pulmonary function among COPD patients.
