摘　　要：目的探讨社区护士对于高危儿过渡期护理的真实体验。方法采用半结构式访谈,对7名社区护士进行访谈,借助Nvivo 10软件,应用Colaizzi现象学研究法分析资料。结果提炼出3大主题:社区护士对高危儿过渡期护理的认识;社区护士实施过渡期护理过程中的感受;实施社区高危儿过渡期护理的困难。结论单纯医院模式或社区模式难以满足高危儿过渡期护理的需求,应根据区域特点建立高危儿医院-社区-家庭过渡期护理模式,组建社区管理团队,实现信息化动态管理,保证医院社区间有效衔接,重视护士培训,开展多方位家长教育,提高高危儿社区护理质量,加强对社区护理的社会支持。Objective To explore the true feeling in community nurses on high-risk infants. Methods Seven community nurses were recruited and interviewed by using semi-structured in-depth interviews method.The Nvivo 10 software was used to analyze the Colaizzi phenomenological data.Results Three main themes were abstracted:community nurses' understanding of transitional care for high-risk infants, community nurses' true feelings during the transitional care process,difficulties in implementing transitional care for high-risk children in the community.Conclusion Simple hospital or community model is difficulty to meet the needs of high-risk infants in transition care, we the hospital-community-family transitional care for high-risk infants should be established,the community management team should be set up to achieve dynamic information management,train nurses,carry out multi-faceted parent education,improve the quality of community care for high-risk infants,and strengthen social support.
