多学科协作护理模式在终末期肾病动静脉内瘘患者中的应用

摘　　要：目的探讨多学科协作护理模式在终末期肾病(ESRD)患者动静脉内瘘(AVF)中的应用效果。方法回顾性分析2016年6月至2017年8月成都中医药大学附属医院肾病科收治的61例ESRD行AVF患者的资料,采用随机数字表法将患者分为试验组(n=31)和对照组(n=30)。试验组采用多学科协作(MDT)护理模式,对照组采用常规护理模式。观察两组患者体温、脉搏、呼吸和血压变化情况;患者满意度、自我护理能力和术后并发症。结果试验组患者护理总满意率明显高于对照组(P<0.05);试验组患者自我护理能力测试量表(ESCA)各项评分明显高于对照组(P<0.05);试验组患者血栓形成、内瘘闭塞及感染发生率明显低于对照组(P<0.05)。结论MDT护理模式可有效提高ESRD行AVF患者的满意度及自我照护能力,减少并发症,延长内瘘使用周期,值得临床推广应用。Objective To explore the application effect of the multi-disciplinary team (MDT) nursing model in arteriovenous fistula surgery (AVF) for patients with end-stage renal disease (ESRD). Methods The data of 61 ESRD patients undergoing AVF from June of 2016 to August of 2017 were retrospectively analyzed. Those patients were randomly divided into the observation group (n=31) and control group (n=30). The observation group was given the MDT nursing model, while the control group was treated with the conventional nursing model. The changes of body temperature, pulse, respiration and blood pressure, patient satisfaction, self-care ability and postoperative complications were observed in the two groups. Results The total satisfaction rate of nursing in the observation group was significantly higher than that in the control group ( P <0.05) and the score of Self-care Ability Scale for the Elderly in the observation group was also significantly higher than that in the control group ( P <0.05). The incidences of thrombosis, internal fistula occlusion and infection in the observation group were significantly lower than those in the control group ( P <0.05). Conclusion The MDT nursing model can effectively improve the satisfaction and self-care ability, reduce complications and prolong the life cycle of internal fistula in ESRD patients, so it is worthy of clinical promotion and application.
