摘  要:目的了解糖尿病足患者居家期间的自我管理认知及体验,为护理人员制订个体化干预方案提供参考及依据。方法2018年8-11月,采用目的抽样法,选取长沙市某三级甲等医院收治的16名糖尿病足患者为研究对象,采用质性研究中的现象学研究方法对其进行深度访谈,根据Colaizzi 7步法对访谈资料进行分析并提炼主题。结果糖尿病足患者居家自我管理认知及体验可归纳为5个主题,即:糖尿病足知识缺乏、足部自护行为欠缺、治疗依从性不高、情绪认知改变、资源利用不完善。结论糖尿病足患者居家期间的自我管理还存在较多问题,医护人员应纠正他们对疾病的不正确认知,强化自我管理行为教育,引导其负性情绪正向转变。此外,开展院外教育及随访教育的干预方案也值得进一步探讨。Objective To understand self-management cognition and experience of diabetic foot patients at home,in order to provide reference and basis for making individualized intervention plan in the future. Methods By purposive sampling method,totally 16diabetic foot patients were conducted from August to November 2018in this study.Phenomenological approach in qualitative research with in-depth interviews was used.Data was analyzed by Colaizzi’s 7-step method.Results Five themes were extracted,including a lack of acknowledge on diabetic foot,weak of foot self-care behaviors,poor treatment compliance,the change of emotion and cognition,inadequate utilization of resources.Conclusions Many problems still exist in self-management of diabetic foot patients at home,so medical staffs have to correct their incorrect cognition of the disease,strengthen the education of self-management behaviors,guide positive change of their negative emotions.In addition,the educational intervention program of outside hospital and follow-up phases should be further discussed.
