百令胶囊联合吗替麦考酚酯胶囊治疗狼疮性肾炎疗效及对免疫学指标和血清IL-18、IFN-γ的影响

摘　　要：目的观察百令胶囊联合吗替麦考酚酯胶囊治疗狼疮性肾炎疗效及对免疫学指标和血清白细胞介素-18(IL-18)、γ-干扰素(IFN-γ)的影响。方法将60例狼疮性肾炎患者随机分为2组,对照组30例给予吗替麦考酚酯胶囊治疗,研究组30例给予百令胶囊联合吗替麦考酚酯胶囊治疗,2组均持续治疗3个月。统计2组治疗后临床疗效,检测2组治疗前后免疫学指标和血清IL-18、IFN-γ水平。结果研究组治疗后临床治疗总有效率显著高于对照组(P<0.05);2组治疗后红细胞沉降率(ESR)、抗双链DNA(ds-DNA)、抗核抗体(ANA)以及血清IL-18、IFN-γ水平均较治疗前显著下降(P均<0.05),且研究组均明显低于对照组(P均<0.05);2组治疗后血清补体C3和补体C4水平均较治疗前显著上升(P均<0.05),且研究组均明显高于对照组(P均<0.05)。结论百令胶囊联合吗替麦考酚酯胶囊治疗狼疮性肾炎疗效确切,同时能明显改善免疫学指标,降低血清IL-18、IFN-γ水平。Objective It is to observe the effect of Bailing Capsule combined with mycophenolate mofetil capsules on lupus nephritis and its effect on immunological indexes and serum interleukin-18 (IL-18) and γ-interferon (IFN-γ). Methods Sixty patients with lupus nephritis were randomly divided into two groups. 30 patients in the control group were treated with mycophenolate capsules, and 30 patients in the study group were treated with Bailing capsules combined with mycophenolate capsules. Both groups were continually treated for 3 months. The clinical efficacy of the two groups was analyzed. The immunological parameters and serum IL-18 and IFN-γ levels were measured before and after treatment. Results The total effective rate of clinical treatment after treatment was significantly higher in the treatment group than that of the control group ( P <0.05). The levels of erythrocyte sedimentation rate (ESR), anti-double-stranded DNA (ds-DNA),anti-nuclear antibody (ANA) and serum IL-18 and IFN-γ were significantly lower than those before treatment in the two groups( P <0.05), and the levels in the study group were significantly lower than those in the control group ( P <0.05);the levels of serum complement C 3 and complement C 4 in the 2 groups after treatment were significantly higher than that before treatment ( P <0.05), and the levels in the study group were significantly higher than that in the control group ( P <0.05). Conclusion Bailing Capsule combined withmycophenolate mofetil capsule is effective in the treatment of lupus nephritis, and can significantly improve the immunological indicators and reduce the levels of serum IL-18 and IFN-γ.
