血必净联合清肺养阴汤对慢性阻塞性肺疾病急性加重期患者炎性反应及抗氧化能力的影响

摘　　要：目的探讨血必净联合清肺养阴汤对慢性阻塞性肺疾病急性加重期患者机体炎性反应及抗氧化能力的影响。方法将86例慢性阻塞性肺疾病急性加重期患者按照随机数字表法分成对照组与治疗组各43例,2组入院后均按照病情给予抗感染、糖皮质激素等常规治疗。治疗组在上述常规治疗基础之上给予血必净静滴与清肺养阴汤口服。2组患者均治疗10d为1个疗程,1个疗程之后评估临床疗效,同时比较2组患者治疗前后血清中炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)、C反应蛋白(CRP)与降钙素原(PCT)]、氧化指标[丙二醛(MDA)与超氧化物歧化酶(SOD)]以及急性生理与慢性健康状况评分(APACHEⅡ评分)的差异。结果治疗组患者治疗总有效率为93.0%,明显高于对照组的76.7%(P<0.05);2组患者治疗后血清中TNF-α、IL-8、CRP、PCT、MDA、SOD水平均较治疗前显著改善(P均<0.05),治疗组各指标改善程度均明显优于对照组(P均<0.05);治疗后2组APACHEⅡ评分均较治疗前明显降低(P均<0.05),且治疗组APACHEⅡ评分明显低于对照组(P<0.05)。结论慢性阻塞性肺疾病急性加重期患者在常规治疗基上增加血必净与清肺养阴汤治疗,可明显提高临床疗效,并且能够有效降低血清中炎症因子水平,提高血清中超氧化物歧化酶水平,大大增强患者机体的抗氧化能力。Objective It is to investigate the effects of Xuebijing combined with Qingfei Yangyin Decoction on inflammatory response and antioxidant resistance in patients with acute exacerbation of chronic obstructive pulmonary disease.Methods 86 patients with acute exacerbation of chronic obstructive pulmonary disease were randomly divided into the control group and the treatment group according to the random number table method,43 patients in each group. All the patients were given anti-infective,glucocorticoid and other treatments according to their condition after hospitalization. On the basis of the above-mentioned conventional treatment,the treatment group was given Xuebijing and Qingfei Yangyin Decoction orally. Both groups of patients were treated for 10 days as a course of treatment,and the clinical efficacy was evaluated after one course of treatment. At the same time,the differences in serum levels of inflammatory factors [tumor necrosis-α (factor TNF-α),interleukin-8 (IL-8),C reaction protein (CRP) and procalcitonin (PCT) ],oxidative indicators [malondialdehyde (MDA) and superoxide dismutase (SOD) ],acute physiology and chronic health status scores (APACHE Ⅱ score) before and after treatment were compared between the two groups. Results The total effective rate was 93. 0% in the treatment group,which was significantly higher than 76. 7% in the control group (P < 0. 05). The levels of TNF-α,IL-8,CRP,PCT,MDA and SOD in the serum of the two groups were significantly improved after treatment in the two groups compared with those before (P < 0. 05),the improvement of each index in the treatment group was significantly better than that in the control group (P < 0. 05). After treatment,the APACHEⅡ scores of the two groups were significantly decreased compared with those before treatment (P < 0. 05),and the scores in the treatment group were better than those in the control group (P < 0. 05).
