涎腺多形性腺瘤和Warthin瘤的多模态超声诊断及Logistic回归分析

摘　　要：目的探讨多模态超声在涎腺多形性腺瘤与Warthin瘤鉴别诊断中的价值。方法选取于我院手术患者75例。组间特点比较用χ2检验,将有统计学意义的参数作为自变量X,以病理结果作为因变量Y,按多模态超声设立3个模型并行Logistic回归分析。结果声像图特点中形状、无回声区、强回声、血流分级、增强强度、增强后分布特征及曲线类型差异有统计学意义(P均<0.05)。3个回归模型均有统计学意义(P均<0.05)。模型Ⅰ、Ⅱ、Ⅲ的灵敏度83.8%、94.6%、97.3%,特异度64.4%、60.0%、84.4%,准确率70.1%、70.2%、86.1%。3种Logistic回归模型的ROC曲线下面积为0.793、0.871、0.935 (P均<0.01)。行多因素亚变量分析,无回声区及增强强度是鉴别的有效参数(P均<0.05)。结论多模态超声对涎腺多形性腺瘤与Warthin瘤的鉴别诊断中具有较高临床应用价值。Objective To evaluate the diagnostic value of multimodal ultrasonic examinations in pleomorphic adenomas and Warthin tumors.Methods A total of 75 lesions were collected.The parameters among groups were compared byχ2 test.Three diagnostic models were established and Logistic regression analysis was conducted.Results There were statistically significant differences in shape,anechoic area,hyper echo,blood flow grade,enhanced intensity,distributive characteristics and curve patterns(all P<0.05).The three Logistic regression models were statistically significant(all P<0.05).By using modelⅠ,Ⅱand Ⅲ,the diagnostic sensitivity,specificity and accuracy were(83.8%,94.6%,97.3%),(64.4%,60.0%,84.4%),and(70.1%,70.2%,86.1%)respectively.The area under the ROC of Logistic equation predicted values were 0.793,0.871,0.935(all P<0.01).Multivariate categorical analysis showed that anechoic area and enhanced intensity were effective diagnostic parameters.Conclusions Multimodal ultrasonic examinations have high clinical value of differentiating salivary tumors.
