超声及超声造影对胆囊较大胆固醇性息肉及腺瘤性息肉的鉴别诊断

摘　　要：目的比较胆囊较大胆固醇性息肉和腺瘤性息肉的超声特征,探讨常规超声及超声造影在二者鉴别诊断中的价值。方法选取经手术病理证实的38例胆固醇性息肉和23例腺瘤性息肉,分析两组病变常规超声和超声造影的特征。结果胆固醇性息肉组与腺瘤性息肉组在病变最大径线、数目、病变内点灶状强回声、造影前后血流可见性及超声造影增强程度、变低增强时间方面比较,差异具有统计学意义(P值均<0.05)。ROC曲线分析结果显示病变最大径线>1.25cm和病变变低增强时间>70s作为诊断腺瘤性息肉的截点值,其诊断的灵敏度较高。结论综合应用高低频超声和超声造影鉴别诊断胆囊较大胆固醇性息肉和腺瘤性息肉具有重要的临床应用价值。Objective To investigate the value of multi-modal ultrasound in the differential diagnosis of larger cholesterol polyps and adenomatous polyps of gallbladder.Methods Patients with lager cholesterol polyps and adenomatous polyps were enrolled.According to the final pathological results,the US and CEUS features were analyzed.Results There were significant differences in size,number,focal spotty echoes,color Doppler blood flow,enhancement degree and time of hypoenhancement of CEUS between the cholesterol polyp group and the adenomatous polyp group(P<0.05).ROC curve analysis showed that the sensitivity were very high when the maximum diameter was greater than 1.25 cm and the time to hypoenhancement exceeded 70 seconds.Conclusions Multi-modal ultrasound was good at distinguishing lager cholesterol polyps and adenomatous polyps of gallbladder.
