益气活血利湿汤对糖尿病视网膜病变模型大鼠炎症因子及视神经保护作用  

Protective Effect of Yiqi Huoxue Lishi Decoction on Inflammatory Factors and Optic Nerve in Diabetic Retinopathy Rats

摘　　要：目的观察益气活血利湿汤对糖尿病视网膜病变模型大鼠炎症因子及视神经保护作用的影响。方法 10只SD大鼠为对照组,40只SD大鼠通过腹腔注射链脲佐菌素和玻璃体腔注射血管内皮生长因子(VEGF)建立大鼠糖尿病视网膜病变模型,再采用随机数字表法分为模型组,益气活血利湿汤高、中、低剂量组,每组10只,模型组和空白对照组给予蒸馏水灌胃,益气活血利湿汤高(2.0g·kg^-1·d^-1)、中(1.0g·kg^-1·d^-1)、低(0.5g·kg^-1·d^-1)剂量组给予益气活血利湿汤溶液煎煮液灌胃。治疗8周后观察大鼠视网膜和视神经结构,采用ELISA检测大鼠血清肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)和白细胞介素-6(IL-6)水平,RT-PCR检测大鼠视网膜神经生长因子(NGF)和核因子-κB(NF-κB)mRNA表达。结果模型组大鼠视网膜出现水肿和扩张充血毛细血管;益气活血利湿汤低剂量组大鼠视网膜水肿,结构紊乱,血毛细血管扩张充,有新生血管;中剂量组视网膜结构相对完整,水肿明显减轻;高剂量组视网膜结构基本完整,未见明显水肿、毛细血管扩张充血和新生血管。模型组大鼠神经纤维排列紊乱,轴突重度肿胀,多处毛细血管扩张出血,胶质细胞增生;益气活血利湿汤低剂量组大鼠视神经纤维排列不规则,轴突肿胀减轻;中剂量组视神经纤维轴突肿胀明显减轻,偶见毛细血管扩张出血和胶质细胞增生;高剂量组视神经纤维排列整齐规则,结构较致密,未见毛细血管扩张出血和胶质细胞增生。与空白对照组比较,模型组大鼠血清TNF-α、hs-CRP和IL-6水平明显升高[TNF-α:(23.46±2.64)pg/mL比(12.34±2.18)pg/mL;hsCRP:(8.69±1.05)pg/mL比(3.64±0.67)pg/mL;IL-6:(97.61±5.12)pg/mL比(31.26±3.26)pg/mL,P<0.05],视网膜NGF和NF-κB mRNA表达显著下调[NGF:(0.27±0.12)比(0.86±0.08);NF-κB:(0.32±0.11)比(0.91±0.09),P<0.05]。与模型组比较,益气活血利湿汤低、中、高剂量组大鼠血清TNFObjective To observe the protective effect of Yiqi Huoxue Lishi Decoction on inflammatory factors and optic nerve in diabetic retinopathy rats. Methods 10 SD rats were selected as the control group. Forty SD rats were injected intraperitoneally with streptozotocin and intravitreal with vascular endothelial growth factor(VEGF)to establish diabetic retinopathy model, and then randomly divided into the model group, high, medium and low dose groups of Yiqi Huoxue Lishi Decoction, 10 rats in each group. The model group and the blank control group was given to distilled water, and the high(2.0 g·kg^-1·d^-1), medium(1.0 g·kg^-1·d^-1) and low(0.5 g·kg^-1·d^-1) dose groups were given to solution decoction of Yiqi Huoxue Lishi Decoction by means of intragastric administration decoction.After 8 weeks of treatment, the retina and optic nerve structure of rats were observed. Serum levels of tumor necrosis factor-α(TNF-α), high-sensitivity C-reactive protein(hs-CRP) and interleukin-6(IL-6) were detected by ELISA. mRNA expression of nerve growth factor(NGF) and nuclear factor-κB(NF-κB) in retina of rats were tested by RT-PCR. Results Retinal edema and dilatation and congestion of capillaries appeared in model group;retinal edema, structural disorder, blood capillary dilatation and neovascularization were observed in low-dose group of Yiqi Huoxue Lishi Decoction;retinal structure of medium-dose group was relatively intact, and edema was significantly reduced;retinal structure of high-dose group was basically intact, and edema, telangiectasia, congestion and neovascularization were not observed. In model group, nerve fibers were disordered, axons were heavily swollen, capillary dilatation and hemorrhage were found in many places, and glial cells were proliferated;in low-dose group, optic nerve fibers were irregularly arranged and axon swelling was alleviated;in medium-dose group, axon swelling of optic nerve fibers was significantly alleviated, occasional capillary dilatation and hemorrhage were observed.
