摘  要:肾脏急性损害是临床最常见的危重急症之一,可由肾脏缺血、肾毒性药物、脓毒血症及泌尿系梗阻等多种疾病引起,临床表现为血清肌酐浓度增加以及少尿,其发病率高、死亡率高,急性肾损伤的死亡率随着急性肾损伤的严重程度的加重而增加。急性肾损伤机制多种多样,线粒体损伤是其发生、发展的重要病理生理学标志之一。急性肾损伤主要影响肾小管,管状细胞富含线粒体,肾小管的线粒体的改变是肾脏疾病发生和发展的重要标志。沉默信息调节因子(SIRTs)是脱乙酰基酶类,该家族成员的功能、作用各不相同,其中SIRT3在线粒体的生成、分裂、融合及自噬中发挥着重要作用。SIRT3参与多种病理生理过程,且在不同原因导致的急性肾损伤中具有保护作用,为急性肾损伤的治疗提供了新思路。Acute kidney injury is one of the most common acute clinical diseases,which can be caused by various diseases,such as renal ischemia,nephrotoxic drugs,sepsis and urinary tract obstruction.The clinical manifestations are increased serum creatinine concentration and oliguria.The incidence and mortality of acute kidney injury are high,and the mortality is increased with the severity of acute renal injury.The mechanism of acute kidney injury is varied,and the mitochondria damage is one of the important pathophysiological markers for the development.Acute renal injury mainly affects the renal tubule.Tubular cells are abundant in mitochondria,the changes of which are important markers for the occurrence and development of renal diseases.Silent information regulators(SIRTs)are a kind of deacetyl-enzyme.The functions and effects are different among different family mumbers,and SIRT3 contributes to the regulation of mitochondrial formation,division,fusion and autophagy.SIRT3 is involved in kinds of pathophysiological processes and has a protective role in acute renal injury,which opens up a new idea for the treatment.
