摘 要:目的探讨放射性脊髓炎的MRI表现特点,评价MRI对放射性脊髓损伤的诊断价值。方法对8头版纳小耳猪行经皮腰椎椎体成形联合125I粒子植入术,术后分别于1、6、12月随即抽取2、3、3头实验猪采用Sonata型1.5T超导核磁机对脊髓行MRI扫描,扫描方法包括常规MRI、增强MRI及弥散加权成像(DWI)。结果病变早期,在T2WI图象上脊髓呈高信号,轴位像上脊髓灰质“H”模糊,与周围白质结构分界不清;T1WI呈稍低信号或未见明显异常信号改变;DWI显示脊髓呈高信号和ADC降低。病变中期,在T1WI和T2WI像上均呈不均匀信号改变。病变晚期,在T1WI和T2WI像上信号变化不明显,软化灶呈长T1长T2改变。轴位T2WI像上可见脊髓灰质“H”和周围白质体积变小,以白质明显。结论MRI是诊断放射性脊髓炎的一种可靠方法。Objective Discussing MRI performance characteristics of radioactive myelites,to evaluate diagnosis value of MRI in radioactive spinal cord injury.Methods 8 small ear pigs of BanNa underwent DSA-guided interventional percutaneous lumbar cone vertebroplasty with I125 injected.Respectively,1、6、12months after surgery we drew2、3、3small ear pigs with Siemens Sonata of 1.5T superconducting MRI machine to scan spinal cord.Scanning methods included ordinary MRI、contrase-enhanced MRI and DWI.Results Lesions in early term,spinal cord showed high signal on T2W1 images.In the axial images,the spinal cord grew matter presented blurried"H".The boundaries between surrounding white matter structure and grey matter were unclear;T1W1 showed a slightly low signal or abnormal signal change;DWI showed a high signal of the spinal cord and ADC decreased.Lesions in medium term,signal in T1W1 and T2W1 both had uneven changes.Lesions in late term,signal in T1W1 and T2W1 changed not obviously.Soften nidus showed a signal of long T1 and T2.In the axial T2W1,the volume of spinal cord grey matter"H"and surrounding white matter became small.But the latter was more obivously.Conclusion MRI is a reliable technique in diagnosing radiation myelitis.
