摘  要:目的评价血清生长分化因子-15(GDF-15)对心力衰竭(HF)诊断的价值。方法检索PubMed、Web of Science、EMBASE等数据库收集文献,起止时间为1996年1月1日至2018年11月30日。应用QUADAS-2评价文献质量,进行敏感性和回归分析探讨异质性因素,通过Stata12.0进行统计分析。结果最终纳入8项研究,HF患者1141例。结果显示GDF-15用于诊断HF的合并敏感度为0.80[95%CI:0.75~0.85,特异度为0.83(95%CI:0.74~0.89),曲线下面积(AUC)为0.87(95%CI:0.84~0.90)。亚组分析显示,GDF-15联合NT-proBNP检测可提高对HF的诊断效能(AUC:0.94);GDF-15在亚洲人群中诊断HF效能优于欧洲人(AUC:0.91VS0.86),检测基于血浆时的效能优于血清(AUC:0.90VS0.50)。结论GDF-15检测对HF有很高的临床辅助诊断价值,且联合NT-proBNP检测有助于提高HF诊断。Objective To evaluate the overall diagnostic efficacy of growth differentiation factor 15(GDF-15)in heart failure(HF).Methods Studies from the databases included PubMed,Web of Science,EMBASE were searched from January 1,1996 to November 30,2018,for eligible studies.Study quality was judged by the QUADAS-2 checklist.Sensitivity and meta-regression tests were performed to assess the causes of heterogeneity.Statistical analyses were conducted via STATA 12.0.Results Eight eligible studies were included,comprising 1 141 HF patients.GDF-15 testing yielded a pooled sensitivity of 0.80[95%confidence interval(CI):0.75~0.85],specificity of 0.83(95%CI:0.74~0.89),and area under the curve(AUC)of 0.87(95%CI:0.84~0.90)in confirming HF.Subgroup analyses showed GDF-15 testing in combined with NT-proBNP could increase the capability in confirming HF(AUC:0.94);GDF-15 testing had better performance in detecting HF in Asians than in Europeans(AUC:0.91 VS 0.86);GDF-15 testing had higher efficacy in plasma than in serum(AUC:0.90 VS 0.50).Conclusion GDF-15 testing is valuable for HF detection,and a combination of NT-proBNP could increase the capability in confirming HF.
