摘 要:目的探究颈动脉超声(CU)联合经颅多普勒超声(TCD)诊断缺血性脑卒中的价值。方法选择我院2017年1月至2019年1月缺血性脑卒中患者90例纳入观察组,另选择同时间段我院因“头痛、头晕”等症状就诊的非缺血性脑卒中患者90例纳入对照组,所有患者均行CU以及TCD检查。比较两组受试者颈动脉斑块情况,颈动脉、颅内动脉血流动力学检测结果,以及CU联合TCD诊断缺血性脑卒中的灵敏度与特异度。结果观察组斑块检出率明显高于对照组,且观察组斑块不规则、低回声者多,对照组均质斑块且规则者多,两组斑块类型与回声特点差异有统计学意义(均P<0.05),两组斑块钙化发生率差异无统计学意义(P>0.05);观察组CU检测颈动脉狭窄率高于对照组,TCD检测显示颅内动脉狭窄率高于对照组,差异均有统计学意义(均P<0.05);联合检测灵敏度与准确度均高于单独CU、TCD检测,差异均有统计学意义(均P<0.05)。结论CU联合TCD具有协同互补的作用,可提高超声检测评价缺血性脑卒中的诊断价值。Objective To investigate the value of carotid ultrasound(CU)combined with transcranialdoppler(TCD)in the diagnosis of ischemic stroke.Methods From January 2017 to January 2019,90 cases of patients with ischemic stroke in our hospital were enrolled in the observation group.At the same period,90 cases of patients with non-ischemic stroke treated with symptoms such as“headache,dizziness”in our hospital were included in the control group.All patients underwent CU and TCD examinations.The carotid plaque,carotid artery,intracranial arterial hemodynamic test results were compared between the two groups.And the sensitivity and specificity of CU combined with TCD in the diagnosis of ischemic stroke of the two groups were compared.Results The detection rate of plaques in the observation group was significantly higher than that in the control group,and the plaques in the observation group were irregular and most were low echoes.The control group had homogeneous plaques whichmost were regular.There were significant differences in plaque types and echo characteristics between the two groups(all P<0.05).There was no significant difference in the incidence of plaque calcification between the two groups(P>0.05).CU detection rate of carotid stenosis is higher in the observation group than that in the control group,TCD detection rate of intracranial stenosis is higher than that of the control group,and the difference was statistically significant(all P<0.05).The sensitivity and accuracy of combined detection were higher than those of CU and TCD alone,and the differences were statistically significant(all P<0.05).Conclusion CU combined with TCD has synergistic and complementary effects,which can improve the diagnostic value of ultrasound detection for ischemic stroke.
