雷珠单抗玻璃体注射联合23G玻璃体切割术治疗增殖型糖尿病视网膜病变临床研究

摘　　要：目的探讨雷珠单抗玻璃体注射联合23G玻璃体切割术在增殖型糖尿病视网膜病变(PDR)患者中应用效果。方法选取我院PDR患者98例,按照随机数字表法分组,对照组49例给予23G玻璃体切割术治疗,观察组49例给予23G玻璃体切割术+雷珠单抗玻璃体注射治疗,比较两组治疗效果及治疗前后血清血管内皮生长因子(VEGF)水平变化情况,并统计两组并发症发生率。结果观察组治疗总有效率为91.84%(45/49),高于对照组75.51%(37/49)(P<0.05);治疗后观察组血清VEGF水平低于对照组(P<0.05);观察组并发症发生率为2.04%(1/49),低于对照组16.33%(8/49)(P<0.05)。结论玻璃体注射雷珠单抗配合23G玻璃体切割术可下调PDR患者血清VEGF水平,降低并发症发生率,效果确切。Objetive To investigate the effect of vitreous monoclonal antibody combined with vitrectomy and 23 G vitrectomy in patients with proliferative diabetic retinopathy(PDR).Methods A total of 98 patients with PDR in our hospital were enrolled according to the random number table.49 patients in the control group were treated with 23 G vitrectomy.In the observation group,49 patients were given 23 G vitrectomy plus ranibizumab intravitreal injection.And the changes of serum vascular endothelial growth factor(VEGF)levels before and after treatment,and the incidence of complications in both groups were counted.Results The total effective rate of the observation group was 91.84%(45/49),which was higher than that of the control group 75.51%(37/49)(P<0.05).The serum VEGF level of the observation group was lower than that of the control group(P<0.05).The complication rate in the observation group was 2.04%(1/49),which was lower than that in the control group(16.33%(8/49))(P<0.05).Conclusion Vitreous injection of ranibizumab combined with 23 G vitrectomy can down-regulate serum VEGF levels in patients with PDR and reduce the incidence of complications.
