摘  要:目的研究5-氨基酮戊酸光动力疗法(ALA-PDT)联合CO2激光治疗难治性尖锐湿疣的价值,为临床诊治提供参考。方法选取2016年12月至2017年12月鄂东医疗集团黄石市中心医院(湖北理工学院附属医院)诊治的168例难治性尖锐湿疣患者作为研究对象。按照随机表法平均分为2组。其中84例为对照组,采用CO2激光治疗;84例为观察组,在对照组基础上结合ALA-PDT治疗。对比治疗后两组患者的临床疗效、总满意率及围治疗期外周T淋巴细胞亚群变化。结果与治疗前相比,治疗后两组患者CD3+、CD4+及CD4+/CD8+等T淋巴细胞亚群增加,且观察组改善情况优于对照组,差异均具有统计学意义(均P<0.05)。两组间相比,观察组治愈率为76.2%高于对照组的54.8%,复发率为23.8%低于对照组的45.2%,差异均具有统计学意义(均P<0.05)。两组间相比,观察组总满意率为95.2%高于对照组的76.2%),差异具有统计学意义(P<0.05)。结论 5-氨基酮戊酸光动力疗法联合CO2激光治疗难治性尖锐湿疣,可有效改善患者免疫功能,提高临床疗效,使复发率降低,且患者满意度高,值得在临床上推广。Objective To study the value of 5-aminolevulinic acid photodynamic therapy(ALA-PDT) combined with CO2 laser in the treatment of refractory condyloma acuminatum(CA), so as to provide references for the clinical diagnosis and treatment. Methods 168 patients with refractory CA in our hospital from December 2016 to December 2017 were selected and divided into 2 groups according to the random table method, each of 84 cases. The control group was treated with CO2 laser, on which basis the observation group was treated with ALA-PDT. The clinical efficacy, total satisfaction degree and changes in peripheral T lymphocyte subsets were compared between the two groups. Results Compared with before treatment, the T lymphocyte subsets of CD3+, CD4+ and CD4+/CD8+ increased in the two groups after treatment, and the improvement in the observation group was more significant than that in the control group, with statistically significant differences(all P<0.05). The cure rate of the observation group(76.2%) was higher than that of the control group(54.8%), while the recurrence rate in the observation group(23.8%) was lower than that of the control group(45.2%), all with statistically significant differences(all P<0.05). The total satisfaction rate of the observation group(95.2%) was higher than that of the control group(76.2%), with statistically significant difference(P<0.05). Conclusions ALA-PDT combined with CO2 laser is effective in the treatment of refractory CA, which can effectively improve the immune function of patients, improve the clinical efficacy and reduce the recurrence rate, with a high degree of patient satisfaction, which is worthy of clinical promotion.
