摘 要:目的研究糖化蛋白与血脂、血糖及肾功能各项生化指标联合检测在糖尿病微血管疾病中的应用价值。方法选取2017年3—12月深圳市宝安区松岗人民医院收治的糖尿病患者3000例为研究对象。将其按照是否合并微血管疾病分为微血管疾病组1620例以及无微血管疾病组1380例。分别比较两组一般资料,各项生化指标水平,并对各项因素与糖尿病微血管疾病的关系予以logisitic回归分析。结果微血管疾病组患者年龄、病程相比无微血管疾病组高(P<0.05)。微血管疾病组糖化蛋白、总胆固醇、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、空腹血糖、餐后2h血糖、尿酸、肌酐以及24h尿微量白蛋白水平相比无微血管疾病组高(P<0.05)。经logisitic回归分析可得:年龄、糖尿病病程、糖化蛋白、总胆固醇、低密度脂蛋白胆固醇、空腹血糖、餐后2h血糖、尿酸、肌酐以及24h尿微量白蛋白水平均是糖尿病微血管疾病的独立危险因素(P<0.05)。结论临床工作中可通过联合检测糖化蛋白与血脂、血糖及肾功能各项生化指标水平,从而有利于早期预测糖尿病微血管疾病的发生,进一步为临床糖尿病微血管疾病的防治提供指导作用。Objective To study the application value of combined detection of glycosylated proteins,blood lipids,blood glucose and renal function in diabetic microvascular diseases.Methods 3 000 patients with diabetes admitted to the hospital from March to December,2017 were studied.The patients were divided into a group of 1 620 cases with microvascular diseases and a group of 1 380 cases without microvascular diseases.The general data of the two groups were compared and the levels of each biochemical index were compared, and the relationship between various factors and diabetic microvascular diseases was analyzed with logisitic regression. Results The age and course of the patients in the microvascular disease group were higher than the non-microvascular disease group (P<0.05).The levels of glycosylated protein,total cholesterol,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol,fasting blood glucose,2 h postprandial blood glucose,uric acid,creatinine and 24 h urinary microalbumin were higher in the microvascular disease group than the non-microvascular disease group(P<0.05).Age,diabetes course,glycosylated protein,total cholesterol,low-density lipoprotein cholesterol,fasting blood glucose,2 h postprandial blood glucose,uric acid,creatinine and 24h urinary microalbumin were independent risk factors for diabetic microvascular diseases(P<0.05).Conclusion The combined detection of glycosylated proteins,blood lipids,blood glucose and renal function in clinical work can help to predict the occurrence of diabetic microvascular diseases at an early stage and provide guidance for the prevention and treatment of diabetic microvascular diseases in clinical practice.
