摘 要:目的探讨类风湿性关节炎患者外周血中Th17/Treg亚群失衡与疾病状态的临床意义。方法选择焦作市第二人民医院2016年4月—2018年4月就诊的类风湿性关节炎患者80例,根据疾病活动度DAS28积分分为轻度组28例,中度组32例,重度组20例,并选择同期40例健康体检人群作为对照组。所有入组人员均于次日清晨,取空腹静脉血5ml,等分为2份分别检测血液中Th17细胞、Treg亚群水平。观察4组受试人群血清中Th17、Treg亚群及Th17/Treg亚群含量,并进行相关性分析。结果全部患者体内Th17百分率及Th17/Treg水平明显高于对照组(P<0.05),全部患者体内Treg亚群百分率明显低于对照组(P<0.05)。重度组、中度组、轻度组体内Th17百分率及Th17/Treg水平由高到低排列(P<0.05),重度组、中度组、轻度组体内Treg亚群百分率由低到高排列(P<0.05)。外周血中Th17、Th17/Treg亚群与疾病活动度DAS28积分呈正相关;外周血中Treg亚群与疾病活动度DAS28积分呈负相关。结论外周血中Th17/Treg亚群失衡可能导致类风湿性关节炎,且对其发病及发展中起到一定作用。Objective To explore the clinical significance of peripheral blood Th17/Treg subsets imbalance in peripheral blood and disease status in patients with rheumatoid arthritis.Methods 80 patients with rheumatoid arthritis who were treated in the hospital from April,2016 to April,2018 were selected and divided into mild group(28 cases),moderate group(32 cases)and severe group (20 cases)according to the disease activity DAS28 score.Another 40 healthy examiners at the same period were selected as control group.All enrolled personnel were taken 5 ml fasting venous blood in the early morning of the next day,and the blood was divided into two parts to determine the levels of Th17 cells and Treg subsets.The contents of Th17,Treg subsets and Th17/Treg subsets in the serum were observed among the four groups,and correlation analysis was done.Results The percentage of Th17 and Th17/Treg level in vivo of all patients were significantly higher than those in control group(P<0.05),and the percentage of Treg subsets in vivo of all patients was significantly lower than that in control group(P<0.05).The percentage of Th17 and Th17/Treg level in severe group, moderate group and mild group were ranked from high to low(P<0.05),and the percentage of Treg subsets in severe group,moderate group and mild group were ranked from low to high(P<0.05).The Th17 and Th17/Treg subsets in peripheral blood were positively correlated with disease activity DAS28 score.Treg subsets in peripheral blood were negatively correlated with disease activity DAS28 score.Conclusion The imbalance of Th17/Treg subsets in peripheral blood may lead to rheumatoid arthritis,and it plays a role in its pathogenesis and development.
