腹腔镜联合纤维胆道镜治疗老年胆总管结石的效果及胃肠功能的影响

摘　　要：目的分析腹腔镜联合纤维胆道镜治疗老年胆总管结石的效果及胃肠功能的影响。方法方便选择该院2013年1月-2019年1月收治100例老年胆总管结石患者,随机分两组,对照组50例使用传统开腹术,研究组50例实施腹腔镜联合纤维胆道镜治疗,对两组手术时间和出血量、胃肠功能指标进行比较。结果研究组患者手术时间和出血量(90.31±7.07)min、(20.96±10.40)mL,各项指标明显比对照组优(t=18.467 9;6.695 2,P<0.05);且研究组患者术后排便时间、肛门排气时间、肠鸣音恢复时间分别为(43.30±12.77)h、(27.96±10.88)h、(13.07±6.51)h,各项指标明显比对照组短(t=5.390 5;4.263 6;5.415 0,P<0.05)。结论腹腔镜联合纤维胆道镜治疗老年胆总管结石患者,不仅能缩短患者手术时间,减少术中出血量,有利于患者术后胃肠功能恢复,临床上值得推广使用。Objective To analyze the effect of laparoscopic combined with choledochoscopy on the treatment of choledocholithiasis and the influence of gastrointestinal function. Methods Convenient select one hundred patients with choledocholithiasis from January 2013 to January 2019 were enrolled in our hospital. They were randomly divided into two groups. 50 patients in the control group underwent traditional open surgery, and 50 patients in the study group underwent laparoscopic combined with choledochoscopy. The operation time, blood loss and gastrointestinal function indexes of the two groups were compared. Results The operation time and blood loss (90.31±7.07) min and (20.96±10.40) mL were significantly higher in the study group than in the control group (t=18.467 9;6.695 2,P<0.05). Defecation time, anal exhaust time, and bowel sound recovery time were (43.30±12.77)h,(27.96±10.88) h,(13.07±6.51) h, and the indexes were significantly shorter than the control group (t=5.390 5;4.263 6;5.415 0,P<0.05). Conclusion Laparoscopic combined with choledochoscopy for the treatment of elderly patients with common bile duct stones can not only shorten the operation time of patients, reduce the amount of intraoperative blood loss, but also facilitate the recovery of postoperative gastrointestinal function. It is worthy of popularization.
