摘 要:目的探讨护士参与的个体化营养干预在癌症患者中的应用效果,为提高癌症患者生活质量提供参考依据。方法采用便利抽样法选取2017年1月—2018年1月间在安徽省第二人民医院接受化疗的癌症患者80例,随机分为对照组和观察组,每组各40例。对照组进行常规饮食指导,观察组采用护士参与的个体化营养干预,比较两组患者营养状况和生活质量。结果干预后6个月,观察组患者白蛋白(64.81±4.73)ng/L、转铁蛋白(29.89±4.11)ng/L、血红蛋白(111.56±8.41)ng/L、淋巴细胞计数(1.71±0.63)×10^9/L,均优于对照组,差异有统计学意义(t值分别为8.112、3.099、2.862、2.138;P<0.05)。干预后6个月,观察组患者躯体状况(PWB)、社会/家庭状况(SWB)评分均优于对照组,差异有统计学意义(P<0.01),两组情感状况(EWB)及功能状况(FWB)方面比较差异无统计学意义(P>0.05)。干预6个月后,观察组患者血液系统、胃肠系统、骨髓抑制、肝损伤4项不良反应发生情况优于对照组,差异有统计学意义(P<0.01)。结论护士参与的个体化营养干预措施能够改善患者的营养状况和生活质量,降低化疗期间不良反应,在长期效应方面具有一定的促进,具有较好的临床推广应用价值。Objective To explore the application effect of nurse participated individualized nutrition intervention in cancer patients, so as to provide evidence for the improvement of the cancer patients' quality of life. Methods A total of 80 cancer patients undergoing chemotherapy in Anhui No.2 Provincial People's Hospital from January of 2017 to January of 2018 were selected by convenience sampling and randomly divided into the control group (n=40) and observation group (n=40). Conventional diet instruction was given in the control group while the nurse participated individualized nutrition intervention was adopted for the observation group, the two groups were compared in terms of their nutritional status and quality of life. Results Six months after intervention, the albumin (64.81±4.73) ng/L, transferrin (29.89±4.11) ng/L, hemoglobin (111.56±8.41) ng/L, lymphocyte count (1.71±0.63)×10^9/L in the observation group were better than those in the control group, and the differences were statistically significant (t=8.112, 3.099, 2.862, 2.138;P<0.05). Six months after intervention, the physical condition (PWB) and social/family status (SWB) scores of patients in the observation group were better than those in the control group with statistical difference (P<0.01), and there was no significant difference in emotional status (EWB) and functional status (FWB) between the two groups (P>0.05). After 6 months of intervention, the incidence of adverse events in blood system, gastrointestinal system, bone marrow suppression and liver injury in the observation group were better than those in the control group with a statistical difference (P<0.01). Conclusions Individualized nutritional intervention participated by nurses can effectively improve the nutritional status and quality of life of patients, plays a positive role in reducing adverse reactions during chemotherapy, promote the long-term effect to a certain extent and has a good clinical application value.
