摘 要:目的了解新疆三级甲等医院外科医务人员职业紧张现状及其影响因素。方法 2018年采取分层整群随机抽样的方法,应用付出-回报失衡(ERI)量表对新疆5所三级甲等医院共计1 000名工龄1年以上的外科医护人员进行调查。结果有效调查对象923人,平均年龄为(31.90±7.18)岁;新疆三级甲等医院医务人员职业紧张检出率为72.20%。不同工龄、夜班频率的外科医务人员在付出维度得分差异有统计学意义(P<0.05);不同工龄的外科医务人员在回报维度得分差异有统计学意义(P<0.05);不同工龄、职称的外科医务人员在内在投入维度得分差异有统计学意义(P<0.05);不同性别、年龄、婚姻状况以及夜班频率的外科医务人员职业紧张检出率差异有统计学意义(P<0.05);Logistic回归分析显示,性别、年龄及夜班频率是职业紧张的独立危险因素。结论本研究群体职业紧张情况较为严重,应予以重视并考虑从个体水平和组织水平开展干预,加强紧张反应的保护因素以缓解外科医务人员的职业紧张现状。Objective To understand the current situation and influencing factors of occupational stress in surgical medical staff in grade-A tertiary hospitals of Xinjiang. Methods By means of stratified cluster random sampling,the pay-to-return imbalance (ERI) scale was applied to investigate a total of 1 000 surgical medical workers with over 1 year working experience in five grade -A tertiary hospitals of Xinjiang. Results There were 923 effective subjects,with an average age of (31.90±7.18) years old. The detection rate of occupational stress for medical staff in grade-A tertiary hospitals of Xinjiang was 72.20%. There were statistically significant differences in the score of payment dimension among surgical medical staff with different length of service and night shift frequency(P<0.05). There was statistically significant difference in the score of return dimension among surgical medical staff with different length of service(P<0.05). There were statistically significant differences in the score of inherent input dimension among surgical medical staff with different length of service and professional title (P<0.05). There were statistically significant differences in the detection rate of occupational stress among surgical medical staff with different gender ,age,marital status and night shift frequency(P<0.05). Logistic regression analysis showed that gender,age and night shift frequency were the independent risk factors of occupational stress. Conclusion The occupational stress of the group in this study is relatively serious,which should be paid attention to,and it is considered that the intervention should be carried out from the individual level and organizational level,and the protective factors of stress response should be strengthened to alleviate the occupational stress of surgical medical staff.
