摘 要:目的分析由人腺病毒3型引起呼吸道传染病聚集性疫情的流行病学和临床特征,为呼吸道传染病疫情的早期判断提供参考。方法对2017年9月26日-11月21日北京市西城区4所小学报告的8起呼吸道聚集性疫情进行现场调查,描述疫情特征,采集患者咽拭子标本,利用荧光定量PCR方法进行病原测定,对部分阳性标本进行病毒分离及基因型别鉴定。结果 8起呼吸道传染病聚集性疫情共涉及68例病例,疫情持续时间平均11.5天,病例病程平均5.4天,患者临床症状以发热(100%)、咽痛(23.58%)和咳嗽(20.69%)为主。基于Hexon,Penton和Fiber 3个基因,构建来源于4所小学的人腺病毒基因亲缘关系树,结果显示,3个基因与人腺病毒3型亲缘关系最近,无重组事件发生且来源于4个学校的腺病毒基因核苷酸同源性为100%。结论 8起疫情均由人腺病毒3型引起,加强聚集性疫情症状及病原学监测,采取有针对性的防控措施,防止疫情进一步扩散和传播。Objective To analyze the epidemiological and clinical characteristics of respiratory aggregated epidemic caused by human adenovirus type 3,provide reference for early judgment of respiratory infectious diseases. Methods A field investigation was carried out on 8 respiratory aggregated epidemics in four primary schools in Xicheng District of Beijing from September 26 to November 21,2017,to describ epidemic characteristics and collect throat swab samples of patients,the fluorescent quantitative PCR was used to detect pathogens,while virus isolation and genotype identification were performed in some positive samples. Results Totally 68 cases were occurred in 8 respiratory aggregated infectious diseases,the average duration of the epidemic was 11.5 days,and the average course of case was 5.4 days. Main symptoms of patients were fever (100%),pharyngalgia (23.58%) and cough (20.69%). The genetic relationship tree of human adenovirus was constructed from four schools basing on genes (Hexon,Penton and Fiber). The results showed that the 3 genes had the closest relationship with human adenovirus type 3. There was no recombination event,and the nucleotide homology of adenovirus gene from four schools was 100%. Conclusion All of 8 respiratory aggregated infectious diseases are caused by adenovirus type 3. It is necessary to strengthen the monitoring of symptoms and pathogens of aggregated infectious diseases and take pertinent preventive and control measures to prevent further spread of epidemics.
