升降理肺汤治疗脓毒症相关性急性呼吸窘迫综合征的临床研究

摘　　要：目的观察升降理肺汤联合西医常规疗法治疗脓毒症相关性急性呼吸窘迫综合征(ARDS)的临床疗效并探讨其作用机制。方法患者60例随机分为治疗组与对照组各30例。对照组均予抗感染、机械通气等西医常规治疗,治疗组在对照组基础上加用中药升降理肺汤。两组疗程均为7 d。比较治疗后两组动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)、氧合指数改变情况及机械通气时间和ICU住院时间。结果两组治疗后PaO2、PaCO2、氧合指数与治疗前比较均改善(均P <0.05);且治疗组各指标疗效均优于对照组(均P <0.01)。治疗组患者机械通气时间和ICU住院时间均短于对照组(均P <0.01)。治疗过程中两组均未出现相关的不良反应。结论升降理肺汤可改善脓毒症ARDS患者肺功能和临床症状,从而缓解病情,改善预后,且安全性良好。Objective:To explore the clinical efficacy of Shengjiang Lifei Decoction combined with conventional Western medicine on sepsis-related ARDS. Methods:60 patients were randomly divided into the treatment group and the control group,30 cases in each. The control group were treated with conventional Western medicine,such as anti-infection and mechanical ventilation. On this basis,the treatment group were treated with Shengjiang Lifei Decoction for 7 days. The changes of partial pressure of oxygen,partial pressure of carbon dioxide,oxygenation index,mechanical ventilation time and ICU hospitalization time were compared between the two groups after treatment. Results:Those were improved after treatment in both groups(all P < 0.05),and the therapeutic effect of each index in the treatment group was better than that in the control group(all P < 0.01). The duration of mechanical ventilation and ICU hospitalization in the treatment group were shorter than those in the control group(all P <0.01). There were no related adverse reactions in both groups during the treatment course. Conclusion:Shengjiang Lifei Decoction can improve the pulmonary function and clinical symptoms of septic ARDS patients,thereby alleviating the condition and improving the prognosis,with good safety.
