摘  要:目的探讨早期应用活血益气中药抑制心梗后心衰大鼠左室重构和心肌细胞凋亡的作用特点。方法采用结扎左冠状动脉法建立心梗后心衰大鼠模型,随机分为模型组、中药组、西药组,每组10只,另选择10只进行假手术作为对照组,对照组和模型组给予0.9%氯化钠注射液灌胃,西药组给予卡托普利灌胃,中药组给予活血益气方灌胃。治疗4周后HE染色观察心脏病理结构,观察心功能指标,放射免疫法检测心肌组织血管内皮细胞生长因子(VEGF)、血管紧张素Ⅱ(AngⅡ)和内皮素-1(ET-1)含量,TUNEL法测定心肌细胞凋亡,RT-PPCR法检测凋亡基因Bcl-2和Bax mRNA水平。结果对照组心肌细胞正常,无坏死,模型组心肌细胞坏死,结缔组织增生,中药组和西药组心肌细胞坏死减少,存在轻微结缔组织增生。中药组和西药组左室发缩内径(LVS)和左室舒张内径(LVD)显著低于模型组,左室射血分数(EDV)和每搏输出量(SV)显著高于模型组(P <0.05),中药组和西药组两组比较差异无统计学意义(P> 0.05)。中药组和西药组VEGF、AngⅡ和ET-1显著低于模型组(P <0.05),中药组和西药组两组比较差异无统计学意义(P> 0.05)。中药组和西药组Bcl-2mRNA水平显著高于模型组Bax mRNA显著低于模型组(P <0.05),中药组和西药组两组比较差异无统计学意义(P> 0.05)。中药组和西药组心肌细胞凋亡数显著低于模型组,且低于对照组(P <0.05),中药组和西药组两组比较差异无统计学意义(P> 0.05)。结论活血益气中药可抑制心梗后心衰大鼠左室重构和心肌细胞凋亡,可能与改善心功能、VEGF、AngⅡ和ET-1表达,调控心肌基因表达有关。Objective:To investigate the effects of early application of Huoxue Yiqi herbs on left ventricular remodeling and myocardial cell apoptosis in rats with heart failure after myocardial infarction. Methods:Rat models of heart failure after myocardial infarction were established by ligating left coronary artery. They were randomly divided into the model group,TCM group and WM group,10 rats in each. Another 10 rats were selected for sham operation as the control group. The control group and model group were given normal saline;WM group was given captopril,and TCM group was given Huoxue Yiqi Decoction. After 4 weeks of treatment,HE staining was used to observe the cardiac structure and cardiac function. VEGF,AngⅡ and ET-1 were detected with RIA,cardiomyocyte apoptosis with TUNEL,the mRNA levels of Bcl-2 and Bax with RT-PPCR. Results:The myocardial cells in the control group were normal without necrosis,while the myocardial cells in the model group were necrotic and the connective tissue proliferated. Myocardial cell necrosis and slight connective tissue hyperplasia were observed in TCM group and WM group. LVS and LVD in TCM group and WM group were lower than that of the model group,and EDV and SV were higher(P < 0.05);there was no significant difference between TCM group and WM group(P > 0.05). VEGF,AngⅡ and ET-1 in TCM group and WM group were lower than that of the model group(P < 0.05);there was no significant difference between TCM group and WM group(P > 0.05). Levels of Bcl-2 mRNA in TCM group and WM group were higher and levels of Bcl-2 mRNA was lower than that of the model group(P < 0.05);there was no significant difference between TCM group and WM group(P > 0.05). Number of apoptotic cardiomyocytes in TCM group and WM group were lower than that of the model group and the control group(P < 0.05);there was no significant difference between TCM group and WM group(P > 0.05).
