血必净注射液联合盐酸氨溴索治疗对老年重症肺炎患者免疫功能和炎性因子的影响

摘　　要：目的观察血必净注射液联合盐酸氨溴索治疗老年重症肺炎的疗效及对免疫功能和炎性因子的影响。方法老年重症肺炎患者192例分为两组,对照组在常规治疗基础上采用盐酸氨溴索,观察组在对照组基础上采用血必净注射液。观察两组治疗14 d后的疗效和治疗前、后CD3+、CD4+、CD8+、CD4+/CD8+、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)及C反应蛋白(CRP)水平。结果与治疗前比较,观察组和对照组治疗后CD3+、CD4+及CD4+/CD8+明显升高(P <0.05),CD8+明显降低,且观察组治疗后改善程度较对照组更明显(P <0.05)。治疗后两组IL-6、IL-8、TNF-α及CRP水平均明显低于治疗前(P <0.05),且观察组治疗后IL-6、IL-8、TNF-α及CRP水平均明显低于对照组(P <0.05)。观察组总有效率明显优于对照组(P <0.05)。结论血必净注射液联合盐酸氨溴索治疗重症肺炎的临床效果较好,可提高患者的免疫功能及减轻炎症反应。Objective:To observe the curative effect of Xuebijing Injection combined with ambroxol hydrochloride on senile severe pneumonia and its influence on immune function and inflammatory factors. Methods:192 patients were randomly divided into the observation group and the control group(n = 96). The control group was given ambroxol hydrochloride on the basis of routine treatment,while the observation group was given Xuebijing on that basis. After 14 days′ treatment,the curative effect of the two groups and the changes in the levels of CD3+,CD4+,CD8+,CD4+/CD8+,IL-6,IL-8,TNF-α and CRP before and after treatment were observed. Results:Compared with before treatment,CD3+,CD4+,CD8+ and CD4+/CD8+ were significantly increased in the observation group and the control group after treatment(P < 0.05);CD8+ was significantly decreased;the changes of the observation group after treatment were more obvious than those of the control group(P < 0.05). After treatment,the levels of IL-6,IL-8,TNF-α and CRP in the observation group and the control group were significantly lower than those before treatment(P < 0.05). Those in the observation group were significantly lower than those in the control group after treatment(P < 0.05). The total effective rate of the observation group was significantly better than that of the control group(P < 0.05). Conclusion:Xuebijing combined with ambroxol hydrochloride has better clinical effect on severe pneumonia,which can improve the immune function of patients and alleviate inflammation.
